Resumo: | A Doença de Alzheimer (DA) constitui uma das demências com maior
dificuldade de diagnóstico, visto que, a sintomatologia surge quando a
doença já está instalada, e com algum grau de progressão. Atualmente,
são desenvolvidos esforços científicos para detetar precocemente a DA
com base em diversos testes cognitivos, exames clínicos e estudos
genéticos, mas também em estudos imagiológicos.
A realização desta investigação tem como objetivo a quantificação e
posterior classificação através da extração de descritores, radiomics
features, para deteção precoce e progressão da DA, partindo da fase
de Mild Cognitive Impairment (MCI) e da fase considerada normal, ou
seja, sem doença. Neste contexto, a Ressonância Magnética (RM) é a
modalidade de imagem que melhor vai permitir a avaliação morfológica
de estruturas subcorticais, nomeadamente, os hipocampos e as
amígdalas, alvo desta investigação.
O dataset foi obtido a partir da plataforma Alzheimer’s Disease
Neuroimaging Initiative (ADNI), e contém 200 estudos de RM para cada
grupo de estudo: Controlo (CN), MCI e DA. Inicialmente, realizou-se a
segmentação das estruturas subcorticais das imagens de RM, seguida
da extração de descritores de volume e texturais dos hipocampos e
amígdalas. Na etapa seguinte, foram analisados estatisticamente os
descritores radiómicos que justificam a predição da evolução da DA e
realizados ensaios de classificação Machine Learning (ML) através do
classificador Random Forest.
Com esta investigação verificou-se que existem evidências claras que
os descritores radiómicos texturais e morfológicos permitem fazer a
distinção entre indivíduos com e sem doença com métricas de
avaliação muito satisfatórias. A transição entre MCI e DA é a que
apresenta um poder classificativo mais baixo, mas ainda assim, com
resultados significativos.
A atrofia surge, em diversos estudos, associada à predição da DA, no
entanto, os nossos resultados identificam alguns descritores radiómicos
texturais como principais na classificação da DA, nomeadamente,
NGLDM_Coarseness, GLRLM_RLNU e GLRLM_GLNU a par dos
descritores de forma (Shape_Volume, Shape_Compacity,
Shape_Spheriecity). Alzheimer's Disease (AD) is one of the most difficult dementias to diagnose, since the symptoms appear when the disease is already installed and with some degree of progression. Currently, scientific efforts are being made to detect AD early on the basis of various cognitive tests and clinical examinations, genetic studies, but also imaging studies. The purpose of this investigation is to quantify and further classify through the extraction of descriptors, radiomics features, for early detection and progression of AD, starting from the Mild Cognitive Impairment (MCI) phase and from the phase considered normal, that is, without disease. In this context, MRI is the imaging modality that will best allow the morphological evaluation of subcortical structures, namely, the hippocampus and the amygdala, the target of this investigation. The dataset was obtained from the ADNI platform and contains 200 MR studies for each study group: Control (CN), MCI and AD. Initially, the transfer of subcortical structures from the MR images was carried out, followed by the extraction of volume and textural features from the hippocampus and amygdala. In the next step, they were statistically analyzed as features that justify the prediction of AD evolution and Machine Learning (ML) classification tests were carried out using the Random Forest classifier. With this investigation, it was verified that there is clear evidence that the textural and morphological radiomic features allow to distinguish between individuals with and without disease with very satisfactory evaluation metrics. The transition between MCI and AD is the one with the lowest ranking power, but even so, with significant results. Brain atrophy appears, in several studies, associated with AD prediction, however, our results identify some textural features as more relevant in the AD classification, namely, NGLDM_Coarseness, GLRLM_RLNU and GLRLM_GLNU along with shape features (Shape_Volume, Shape_Compacity, Shape_Spheriecity). |
