Resumo: | No âmbito do 3º semestre do 2º ano do Curso de Mestrado em
Enfermagem de Saúde Familiar foi desenvolvido um estágio de
natureza profissional na Unidade de Cuidados de Saúde
Personalizados Águeda V (ACeS Baixo Vouga), por se considerar ser
o contexto de excelência para o exercício das competências do
enfermeiro de família. Esta unidade funcional serviu de contexto à
realização de um trabalho de investigação baseado na problemática
da adesão ao regime terapêutico nos utentes com Diabetes Mellitus
(DM) tipo 2. A natureza crónica da DM tipo 2, a necessidade vitalícia
de medicamentos, a exigência de mudanças no estilo de vida, o
sofrimento social, cultural e psicológico que lhe está inerente, as
manifestações clínicas e as complicações associadas justificam a
complexidade da gestão da mesma e aumentam o risco da não
adesão. Conhecer o nível de adesão ao regime terapêutico do
indivíduo é fundamental tanto para o planeamento de cuidados como
para a adequação de estratégias na resolução de problemas.
Assim, este relatório descreve e analisa critico-reflexivamente as
competências desenvolvidas, dificuldades e estratégias para as
ultrapassar, bem como, apresenta os resultados do estudo de
investigação sobre a avaliação do impacto da intervenção do
enfermeiro de família na adesão ao regime terapêutico dos utentes
com DM tipo 2 seguidos na UCSP Águeda V.
Este estudo é de natureza quantitativa com aplicação de apenas um
ciclo de investigação-ação, sendo que após um diagnóstico inicial, se
implementou uma intervenção psicoeducativa de modo a verificar o
seu efeito no nível de adesão ao regime terapêutico. Na avaliação foi
utilizada a escala de Avaliação do Autocuidado na Diabetes (AAD), a
Medida Psicométrica de Adesão aos Tratamentos (MPAT) e ainda, um
questionário de caracterização sociodemográfica e clínica dos
participantes em estudo.
A amostra foi constituída por 102 utentes com uma média de idades
de 67,5 anos, maioritariamente do sexo masculino (56,9%), com o
diagnóstico de DM tipo 2 entre 6 a 10 anos e predominantemente com
hemoglobina glicada inferior ou igual a 7%. Os participantes mostraram
maior nível médio de adesão na medicação e menor na atividade física.
Após a implementação da intervenção psicoeducativa observaram-se
diferenças estatisticamente significativas no nível médio de adesão em
todas as atividades do autocuidado. No que se refere à proporção de
indivíduos com mudança de classificação de “não aderente” para
“aderente”, esta mostrou-se estatisticamente significativa apenas para
a alimentação geral e em relação à medicação, nomeadamente nos
aspetos relacionados com o esquecimento da toma e descuido das
horas da mesma.
Com a realização do estágio, e integrado no seu desenvolvimento a
concretização do trabalho de investigação foram desenvolvidas as
competências esperadas e alcançados os objetivos delineados
previamente. Conclui-se com o estudo que a intervenção desenvolvida
foi relevante para a adesão ao regime terapêutico nos utentes com DM
tipo 2. In the 3rd semester of the 2nd year of the Master in Family Health Nursing, a professional internship was developed in the Personalized Health Care Unit Agueda V (ACeS Baixo Vouga) once it is considered to be the context of excellence for a family nurse to exercise competencies. This functional unit was an appropriated context for conducting a research work based on the problem of adherence to the therapeutic regimen by patients with Diabetes Mellitus (DM) type 2. The chronic nature of DM type 2, the lifelong need for medication, the requirement for changes in the lifestyle, the social, cultural and psychological suffering inherent to it, the clinical manifestations and the associated complications justify the complexity of the treatment management and confirm the risk of non-adherence. To know the level of adherence to the therapeutic regimen by the individual is fundamental both for the planning of care and for the adaptation of strategies in problem solving. Hence, this research work describes and analyses in a criticalreflectively way the developed competencies, the difficulties felt and the strategies to overcome them, as well as, it presents the research results about the evaluation of the impact of the family nurse intervention in the adherence to the therapeutic regimen by patients with DM type 2 monitored at the Personalized Health Care Unit Agueda V. This is a quantitative study with the application of only one cycle of the research-action methodology, and after an initial diagnosis, a psychoeducative intervention was implemented in order to verify its effect on the level of adherence to the therapeutic regimen. To evaluate it was used the Self-Care Diabetes Assessment and the Psychometric Measure of Adherence to Treatment, as well as a sociodemographic and clinical characterization questionnaire of the studied participants. The sample was made up of 102 individuals being the average age of 67.5 years old, mainly males (56,9%), with DM type 2 diagnosed 6 to 10 years ago, mostly with less than or equal to 7% glycated haemoglobin. The patients showed a higher average level of medication adherence and lower physical activity. After the implementation of the psychoeducative intervention there were statistically significant differences in the average level of adherence in all self-care activities. Regarding the proportion of individuals with a change from "non-adherent" to "adherent" classification, this was statistically significant only for general nourishing and medication in aspects related to obliviousness and carelessness of the time of intake. With the attainment of the internship, and because of its development the accomplishment of the research work, the expected competencies were developed and the objectives previously outlined were achieved. We can conclude with this study that the intervention developed was relevant for the adherence to the therapeutic regimen by the patients with DM type 2. |
