Abstract: | A infeção por citomegalovírus (CMV) é mais frequente em países
em desenvolvimento e em comunidades com estatuto
socioeconómico mais baixo, e representa a causa viral mais
significativa de malformações de nascimento em países
industrializados. É possível reduzir os danos decorrentes desta
infeção se forem seguidas e aplicadas algumas medidas de
prevenção.
Este trabalho teve como objetivo determinar a prevalência de
infeção por citomegalovírus nas utentes que recorreram à consulta
de obstetrícia do Centro Hospitalar de Vila Nova de
Gaia/Espinho,EPE, entre 8 de Fevereiro de 2001 e 15 de Dezembro
de 2010. Para esse efeito, foi realizado um estudo estatístico
utilizando a base de dados deste Centro Hospitalar relativos à
consulta em causa e aos valores dos respetivos pedidos do CMV.
Verificou-se que 82,8% das utentes tinham IgG anti-CMV positiva e
16,9% apresentavam resultados negativos para a IgG anti-CMV.
Em relação à IgM anti-CMV 97,4% tinham resultado negativo e
0,9% eram positivas. Foi feita a avidez da IgG anti-CMV das
utentes que apresentavam valores positivos e duvidosos para a IgM
anti-CMV. Observou-se que 65,8% tinham uma avidez alta (infeção
com mais de 3 meses) e 0,5% avidez baixa (infeção recente, com
menos de 3 meses). Relativamente à avidez da IgG anti-CMV
33,7% dos resultados foram inconclusivos, o que não permitiu
determinar se a infeção era recente ou antiga. Observou-se ainda
que 0,3% (19 utentes) seroconverteram durante o período de
estudo.
Os pedidos de serologia para CMV no acompanhamento das
mulheres em idade fértil acarretam custos consideráveis. Os
resultados deste estudo indicam que estas despesas são
justificáveis pois uma fracção elevada das mulheres estudadas
(83%) está infectada e foi detectada seroconversão em 19
pacientes durante o período de estudo. À alta prevalência da
infeção está associado o risco de reinfeção ou reativação. A
seroconversão das utentes de IgG anti-CMV negativa para positiva
associada à baixa avidez das IgG, é a melhor forma de documentar
uma infeção primária e permitir ao clinico a decisão de realizar a
colheita de líquido amniótico para atempadamente determinar
possíveis fetopatias graves. Cytomegalovirus infection (CMV) is more frequent in developing countries and communities with lower socio-economic status, representing and the same time the most significant viral cause of birth defects in industrialized countries. It is possible to reduce the damage caused by this infection if some prevention measures are applied. This work aimed to determine the prevalence of cytomegalovirus infection in users of the Obstetrics Consult of the Hospital Center of Vila Nova de Gaia / Espinho, EPE, between February 8, 2001 and December 15, 2010. To reach this goal a statistical study was perfomed using the database of the Hospital Center and the values of the respective CMV tests. It was concluded that 82.8% of the users had anti-CMV IgG positive and 16.9% had negative results for anti-CMV IgG. With regard to anti-CMV IgM 97.4% were negative and 0.9% were positive. It was evalueted the avidity of the anti-CMV IgG for patients with positive values and equivocal anti-CMV IgM.It was observed that 65.8% had a high avidity (infection with more than 3 months) and 0.5% had low avidity (recent infection with less than 3 months). Thirty three percent of anti-CMV IgG avidity test were inconclusive, wich did not allow establish if the infection was recent or old. It was also observed that 0.3% seroconverted during study period. . Requests for CMV serology in monitoring women of childbearing age entail considerable costs. The results of this work show that these costs are, however, justified because a high fraction of women (83%) are, infected and it was detected seroconversion in 19 patients during the study period. To the high prevalence of the infection is associated with the risk of reinfection or reactivation. The seroconversion from anti-CMV IgG negative to positive associated with low avidity of IgG, is the best way to document a primary infection and allows the clinician to make a decision relatively to amniotic fluid collection in order to determine serious fetal pathologies. |
