Contributos da CIF na avaliação da disfagia no pós-AVC

Abstract

O Acidente Vascular Cerebral (AVC) é uma das principais causas de lesões permanentes em adultos, estando a disfagia presente em cerca de 40-50% dos indivíduos, durante a fase aguda. Sendo a disfagia uma perturbação que pode resultar em morte, a sua abordagem segue essencialmente o modelo médico. Contudo, quando esta é observada através da Classificação Internacional de Funcionalidade, Incapacidade e Saúde (CIF), é possível expandir e melhorar avaliações e intervenções junto de indivíduos com disfagia. O presente trabalho tem como objetivo contribuir para a criação de uma nova metodologia de avaliação no ambiente da CIF, orientada especificamente para a disfagia no pós-AVC. Essa metodologia designa-se Protocolo Disfagia no pós-AVC e divide-se em duas partes: a primeira constituída pela versão reduzida do Core Set da CIF para o AVC, e a segunda por categorias relativas à deglutição. Os resultados (da criação do protocolo e da sua administração a 32 doentes com AVC), são indicativos de que o instrumento criado permite descrever disfagia decorrente de AVC e apresenta boas medidas de validade e medidas de fiabilidade razoáveis. Considera-se ter sido dado um importante contributo para o crescente conhecimento ao nível da operacionalização da CIF, em especial do seu uso na prática clínica.

Stroke is one of the main causes of permanent lesions in adults, being dysphagia present in 40-50% during the acute phase. Dysphagia is a disorder that could potentially result in death, what makes is management mostly guide by the medical model. However, when dysphagia is examined by the International Classification of Functioning, Disability and Health (ICF), it is possible expand and greatly enhance assessments an interventions for persons with dysphagia. The present work aims to contribute to the developed of a new methodology for assessment, according to the assumptions of ICF, specifically geared to dysphagia after stroke. This methodology is called Dysphagia After Stroke Protocol and is divided into two parts: the first consist on the Brief Core Set for Stroke and the second on ICF categories for deglutition. The results (development of the protocol and its administration to a group of 32 patients with stroke) indicates that the created instrument it’s useful to describe dysphagia after stroke and has good values of validity and reasonable values of reliability. With this work, another step towards the knowledge of the operationalization of the ICF has been made, especially within clinical practice.
