Abstract: | O presente relatório expõe as atividades desenvolvidas no estágio de
natureza profissional, no âmbito do 3º semestre do 6º curso de Mestrado
em Enfermagem de Saúde Familiar da Universidade de Aveiro, realizado
na Unidade de Cuidados de Saúde Personalizados (UCSP) Águeda II do
Agrupamento de Centros de Saúde do Baixo Vouga. Teve como objetivos
descrever e realizar uma análise crítico-reflexiva das competências
desenvolvidas durante o estágio e apresentar um estudo de investigação
intitulado “O enfermeiro de família, o cuidador familiar e a perda ambígua:
tradução, adaptação e validação da Boundary Ambiguity Scale”. O estudo
observacional, transversal, descritivo e correlacional e teve como objetivo:
traduzir, adaptar culturalmente e validar a Boundary Ambiguity Scale para
a população portuguesa A amostra foi constituída por 88 cuidadores
familiares de Pessoa com Demência (PcD) da lista de utentes da UCSP
Águeda II, UCSP Águeda V e Unidade de Saúde Familiar (USF) Águeda
+ Saúde. A validade foi avaliada pelo Índice de Validade de Conteúdo
(IVC) e Análise Fatorial Exploratória (AFE). A consistência interna foi
determinada pelo Coeficiente ômega de McDonald (ω). Na análise
estatística foram utilizados testes não-paramétricos e o nível de
significância foi de p<0.05. Estudo aprovado pela Comissão de ética da
Administração Regional de Saúde Centro (processo nº11/2021).
Após o processo de tradução, a escala foi avaliada por um grupo de peritos
para determinar o IVC. Foram realizadas três rondas, sendo os resultados
finais excelentes (IVC≥0,8). Na AFE identificou-se um modelo com dois
fatores, que explicam 44% da variância. Os itens do fator A apresentaram
uma carga fatorial entre 0,51 e 0,81 e um ω de 0,79. Os itens do fator B
apresentaram uma carga fatorial que variou entre 0,39 e 0,62 e um ω de
0,62. Os valores médios da Escala de Limites Ambíguos (ELA) foram de
2,99, no fator A e B foram de 3,46 e 2,37. A amostra foi constituída por 88
cuidadores informais, 75% do sexo feminino, com idade média de 62,84 e
com baixa escolaridade (0-4 anos-42%; 5-12 anos-44,3%). A maioria
reside próximo da PcD (79,5%), não tem vínculo laboral (61,4%) e não
recebe nenhum apoio (59,1%). Não se verificaram diferenças
estatisticamente significativas entre o género e ELA. No grau de
parentesco verificou-se uma diferença estatisticamente significativa com a
ELA. A nível descritivo a ordenação média relativa aos cônjuges foi a mais
elevada. Verificou-se uma diferença estatisticamente significativa com a
ELA e o fator A, revelados em valores mais elevados no grupo dos
cuidadores familiares que vivem com a PcD. Não se verificaram diferenças
estatisticamente significativas entre a ELA, fator A, fator B e o estado civil
do cuidador familiar. Pessoas com menor escolaridade, tiveram maiores
níveis de perda ambígua. A situação profissional apresentou diferença
estatisticamente significativa para o fator A e valores mais elevados nos
cuidadores familiares com vínculo laboral. Não se verificaram diferenças
estatisticamente significativas em relação a ter apoio domiciliário. Houve
correlação significativa positiva fraca com a ELA e o fator A, em relação à
idade. Da correlação entre a ELA e a Escala de Perceção de Stresse
evidenciou-se uma correlação significativa positiva e moderada.
Os resultados deste estudo sugerem que a ELA é um instrumento com
propriedade psicométrica de fiabilidade e validade aceitáveis. A ELA
constituiu-se, assim, num instrumento válido a ser aplicado quer no âmbito
da investigação, quer na prática clínica, podendo a sua utilização ser
relevante na compreensão dos sentimentos vivenciados pelos cuidadores
familiares de PcD. This report presents the activities developed in the professional internship, within the 3rd semester of the 6th Master's course in Family Health Nursing at the University of Aveiro, held at UCSP Águeda II (ACeS Baixo Vouga). The aims of this report were: describe and carry out a critical-reflective analysis of the skills developed during the internship and present a research study entitled “The family nurse, the family caregiver, and ambiguous loss: translation, adaptation and validation of the Boundary Ambiguity Scale”. The observational, cross-sectional, descriptive, and correlational study, that aims to: translate, culturally adapt and validate the Boundary Ambiguity Scale (BAS). The sample consisted of 88 family caregivers of Person with Demencia (PwD) from the list of users at UCSP Águeda II, UCSP Águeda V and USF Águeda + Saúde. Validity was assessed using the Content Validity Index (CVI) and Exploratory Factor Analysis (EFA). Internal consistency was determined by the McDonald's omega coefficient (ω). In the statistical analysis, non-parametric tests were used and the significance level was p<0.05. Study approved by the Ethics Committee of the Regional Health Administration Center (process nº 11/2021). After the translation process, the scale was evaluated by a group of experts to determine the CVI. Three rounds were carried out, the final results being excellent (CVI≥0,8). In EFA, a model with two factors was obtained, which explains 44% of the variance. The items in factor A had a factor loading between 0.51 and 0.81 and a ω of 0.79. Factor B items had a factor loading that ranged between 0.39 and 0.62 and a ω of 0.62. Mean BAS values were 2.99, for factor A and B they were 3.46 and 2.37. The sample consisted of 88 informal caregivers, 75% females, mean age of 62.84 years, and with low education (0-4 years-42%; 5-12 years-44.3%). The largest lives close to the PwD (79.5%), have no employment relationship (61.4%) and does not receive any support (59.1%). There were no statistically significant differences between gender and BAS. In the degree of relatedness, there was a statistically significant difference with BAS. At a descriptive level, the average ranking relative to spouses was the highest. There was a statistically significant difference with BAS and factor A, revealed in higher values in the group of family caregivers living with PwD. There were no statistically significant differences between BAS, factor A, factor B and the marital status of the family caregiver. People with less education had higher levels of ambiguous loss. The professional situation showed a statistically significant difference for factor A and higher values for family caregivers with a labor contract. There were no statistically significant differences in terms of having home support. There was a weak significant positive correlation with BAS and factor A, in relation to age. The correlation between BAS and the Perception of Stress Scale showed a significant positive and moderate correlation. The results of this study suggest that BAS is an instrument with psychometric properties of acceptable reliability and validity. Therefore, BAS constituted a valid instrument to be applied both in research and in clinical practice, and its use could be relevant in understanding the feelings experienced by family caregivers of PwD. |
