Vivências na transição para a parentalidade: o contributo do enfermeiro de família

Abstract

O Enfermeiro Especialista em Enfermagem de Saúde Familiar (EEESF) presta cuidados à família nos mais variados contextos e em diversas fases do ciclo de vida. Este apresenta conhecimentos e competências que facilitam o trabalho colaborativo com a família, de forma a facilitar a reorganização da sua estrutura, na definição de novos papéis e no desenvolvimento de novas competências, durante as transições normativas, situacionais ou nos processos saúde-doença, no seu ciclo vital familiar. A elaboração deste relatório permitiu refletir sobre as competências específicas inerentes ao EEESF e descrever o trajeto realizado para a aquisição das mesmas ao longo do estágio. Integrado no mesmo foi desenvolvido um estudo empírico sobre os contributos do Enfermeiro de Família (EF) na transição para a parentalidade vivenciada pela mulher, focado na prestação de cuidados de enfermagem à família em transição, que teve como objetivos: conhecer as vivências da puérpera quanto à transição para a parentalidade e identificar a relevância atribuída ao contributo proporcionado pelo EF, neste processo transacional, durante as Consultas de Vigilância da Gravidez (CVG). O estudo foi realizado na USF Águeda + Saúde, entre o mês de novembro de 2017 e fevereiro de 2018 e enquadra-se numa abordagem fenomenológica, segundo a metodologia qualitativa, cuja amostra intencional foi constituída por 14 puérperas que cumpriam os critérios de inclusão. A recolha dos dados realizou-se através de entrevistas semiestruturadas, como forma de obter narrativas das vivências da mulher durante a gravidez e puerpério. Dos dados obtidos foi feita análise de conteúdo tendo como suporte o software webQDA®. Os resultados obtidos descreveram a transição para a parentalidade como a passagem para o papel de “tornar-se mãe”, acompanhada por diversas emoções, dúvidas e preocupações, cujas fontes de apoio à mulher foram o companheiro e o EF. Assim, o contributo do EF foi descrito como fonte de apoio emocional e educacional, integrante da rede de apoio à grávida/casal e à família e agente facilitador no processo de transição para a parentalidade, no decurso das CVG e na visita domiciliária pós-parto. A relação terapêutica estabelecida foi de excelência, tendo sido promovido o envolvimento paterno na maioria das vezes. A família foi considerada como um sistema, tendo sido promovida a (re)organização familiar face às adaptações subjacentes no ciclo vital familiar. Todos os cuidados prestados na vigilância, na promoção e na manutenção da saúde e do bem-estar da mulher e sua família são, na sua pluralidade, concordantes com as suas expectativas e necessidades. Estes resultados são congruentes com o que é preconizado pelo Programa Nacional de Vigilância da Gravidez de Baixo Risco e pela Ordem dos Enfermeiros

The Family Health Nursing Specialist Nurse provides care to the family in the most varied contexts and at various stages of the life cycle. It presents knowledge and skills that facilitate the collaborative work with the family, in order to facilitate the reorganization of its structure, the definition of new roles and the development of new competencies, during normative, situational transitions or in the health-disease processes, in their family life cycle. The elaboration of this report allowed to reflect on the specific competences inherent to the family health nursing specialist nurse and to describe the path taken to acquire them during the internship. An empirical study was carried out on the contributions of the family nurse in the transition to parenting experienced by the woman, focused on the provision of nursing care to the family in transition, with the following objectives: to know the puerpera experiences about the transition to parenting and to identify the relevance attributed to the family nurse contribution in this transactional process, during the pregnancy monitoring appointments. The study was carried out at USF Águeda + Saúde, between November 2017 and February 2018, and is part of a phenomenological approach, according to the qualitative methodology, whose intentional sample consisted of 14 puerperas who meet the inclusion criteria. The data collection was done through semi-structured interviews, as a way of obtaining narratives of women's experiences during pregnancy and puerperium. From the obtained data a content analysis was performed, having the software webQDA® as support. The results obtained describe the transition to parenting as the transition to the role of "becoming a mother", accompanied by several emotions, doubts and concerns, whose sources of support to the woman were the companion and the family nurse. Thus, the contribution of family nurse was describe as having been a source of emotional and educational support, integral part of the support network for the pregnant/couple and the family and as a facilitating agent in the process of transition to parenthood, during the pregnancy monitoring appointments and in the puerperal home visit . The established therapeutic relationship was of excellence in its majority, having been promoted the paternal involvement in the majority of the times and considered the family with system, attending to the stage of the developmental cycle which it was, promoting the (re)organization of the family. All care provided in the monitoring, promotion and maintenance of the women and their families health and well-being are, in their plurality, consistent with their expectations and needs. These results are congruent with what is advocated by the National Program for Low-Risk Pregnancy Surveillance and by the Nurses' Order
