Abstract: | O índice de envelhecimento da população portuguesa tem vindo a
aumentar exponencialmente, sendo uma realidade à semelhança dos
restantes países desenvolvidos. A este fenómeno ocorre em
simultâneo, o envelhecimento de filhos com T21, o que constitui um
desafio para os casais idosos portugueses. Por ser este um fenómeno
ainda recente, há ainda na literatura uma lacuna considerável de
estudos na área, com destaque para Portugal. Neste sentido, a
presente investigação, teve como objetivo geral conhecer as
experiências de casais portugueses que estão a envelhecer a cuidar
de filhos com T21. Este estudo foi desenvolvido com base na
abordagem qualitativa, através do método da história oral de Paul
Thompson. Os dados foram recolhidos através de entrevistas
semiestruturadas, com catorze casais portugueses, com mais de 60
anos, com filhos com T21. A maioria dos casais foi recrutada através
de instituições de apoio a crianças com deficiência e os restantes
através de snowbowl. Os princípios éticos foram tidos em
consideração durante o processo da investigação. A análise dos
dados foi desenvolvida através da análise de conteúdo, com o apoio
do programa WebQda. Este estudo abrangeu três estudos sobre as
diferentes fases do ciclo vital, o nascimento dos filhos com T21, a
infância e adolescência dos filhos com T21, e a adultez dos filhos com
T21. O primeiro estudo descreve o impacto de ter um filho com T21,
em Portugal. Assim, o que ressalvou foi a notícia de um filho com T21,
na totalidade dos casais, acontecer após o parto. A falta de atenção
e/ou preparação dos profissionais de saúde no apoio e na informação
prestada aos pais, após o nascimento de seu filho, não constituindo
um elo de ligação entre estes casais e o sistema de saúde. Quanto
ao segundo estudo, que aborda as fases da infância e adolescência,
as experiências foram pautadas por desafios constantes inerentes ao
crescimento e adaptação à realidade de seus filhos com T21. A
inclusão no ensino regular, a institucionalização precoce e as
alterações tanto corporais como comportamentais, foram alguns dos
tópicos abordados pela sua importância no seio familiar. Neste
sentido, o estudo menciona que apenas cinco crianças puderam
atender a sua inclusão no ensino regular, enquanto os restantes ou
foram institucionalizados ou ficaram ao encargo dos progenitores.
Relativamente à aprendizagem cognitiva, tanto as crianças que
frequentaram o ensino regular como a institucionalização, não
conseguiram atingir esta meta de ensino. Esta fase também se pautou
pela descriminação no âmbito educacional, familiar e social. Na
decorrência do crescimento, alterações comportamentais foram
surgindo e os casais foram superando com estratégias encontradas.
Nas alterações corporais, o estudo demonstrou a falta de preparação
dos progenitores pois somente focaram os aspetos, que
consideraram mais emergentes, tais como o ciclo menstrual e o
aparecimento da barba. No terceiro estudo sobre a adultez dos filhos
dos casais com filhos com T21, os resultadas apontaram a falta de
literacia de todos os filhos, pela decorrência de seus percursos
escolares e a escassez de apoios prestados, limitando-os na
construção de seu futuro. À data do estudo todos os filhos adultos
coabitavam com seus pais, alternado a sua vida entre a habitação
familiar e a instituição. Também ficou evidente que apesar da
deficiência, os filhos apoiavam nas lides domésticas minimizando a sobrecarga parental. Nesta etapa, o estudo evidencia a
desvalorização dos casais quando se menciona o futuro de seus
filhos, quer pela falta de confiança nos setores públicos, quer pelas
suas próprias condições físicas. Assim, os resultados permitiram
concluir que tantos os casais como os filhos com T21 tiveram um
percurso de vida desafiador. A escassez de informação e apoio,
nomeadamente de profissionais da área da saúde e educação,
sobressaiu ao longo das narrativas dos casais. Apesar das políticas
já existentes, direcionadas a pessoas com T21, a descriminação
ainda persiste. Deste modo, torna-se urgente medidas locais e
globais para que as pessoas com T21 e seus progenitores possam
ter as suas necessidades gerais atendidas, e exercer plenamente os
seus direitos de cidadania. Considerando a importância do tema e as
lacunas de conhecimento na área, novos estudos são necessários,
especialmente em Portugal, para compreender as necessidades
geriátricas, tanto dos casais como dos filhos com T21, de modo a que
as intervenções a este grupo social sejam céleres e realistas. The Portuguese population ageing index, as in other developed countries, has increased exponentially. The same phenomenon occurs with children with T21, which is a challenge for Portuguese elderly couples. Since this is still a recent phenomenon, there is still a considerable gap in the literature of studies in this area, particularly in Portugal. In this sense, this research aims to know the Portuguese couples’ experiences that are ageing, and having to take care of children with T21. This study was developed based on a qualitative approach, using Paul Thompson’s oral history method. Data was collected through semi-structured interviews with fourteen Portuguese couples aged at more than sixty years who had children afflicted with T21. Most couples were recruited from institutional support system for disabled children and the remaining ones through ‘Snowball’. Ethical principles were taken into consideration during the investigation process. Data analysis was carried out based on content analyses with the support of the WebQda programme. This research included three separate studies on the different stages of the life cycle, the birth, the childhood and adolescence and the adulthood of children with T21. The first study describes the impact of having a child with T21 in Portugal. What stood out was the news of the child diagnosed only after birth. The lack of attention/preparation of health professionals regarding the support and information provided to parents, after the birth of their child, leads to a lack of connection between these couples and the health system. The second study, covering childhood and adolescence stages, revealed constant challenges on adapting to the growth and reality of their T21 children. Inclusion in mainstream education, institutionalisation and both physical and behavioural changes, were some of the topics covered regarding its importance to the family. In this sense, the study reveals that only five children were able to undergo a normal mainstream education, while the rest were either institutionalised or left in the in charge of their parents. With regard to cognitive learning, both children who attended regular schools and those attending institutions failed to achieve their educational goals. This phase was also marked by discrimination in educational, family and social contexts. Increasing behavioural changes due to the natural growth process were emerging and couples were trying to overcome it by finding some strategies as they were occurring. Regarding changes in the body, the study demonstrated a lack of parental preparedness as they only mentioned the most emerging aspects, such as the menstrual cycles and facial growth. In the third study, focused on the adulthood of children with T21, the results pointed out a lack of literacy in all of them, due to their educational background and lack of support provided, which limited their ability to build a future. At the time of the study, all affected children who had reached adulthood lived with their parents and alternated their lives between the family home and institutions. It was also evident that despite their disabilities, the children were supportive with house work, minimising parental overload. At this stage, the study highlighted the devaluation of couples when it referred to their children’s future, either by lack of confidence in the public services or by their own physical condition. The concluding results showed that not only their parents but also children affected by T21 had a challenging life. What stood out the most in couples’ narratives was the lack of information and support, namely from the professionals in the health and education sectors. Despite the existing polices aimed at people with T21, discrimination still persists. Thus, it is imperative that local and global measures for people with T21 and their parents have their general needs met in accordance with their rights as citizens. Considering the importance of the issue and lacking knowledge in this area, further studies are needed, especially in Portugal, to understand the geriatric needs of both couples and their children with T21, so that interventions regarding this social group are tackled with urgency and realism. |
