Significados do cuidado de enfermagem à pessoa idosa em cuidados intensivos

Abstract

O presente estudo teve como objectivo conhecer os significados do cuidado de enfermagem à pessoa idosa numa unidade de cuidados intensivos (UCI) de um hospital da região centro de Portugal, a partir das vivências de enfermeiros (as). Foi desenvolvido através da abordagem qualitativa, utilizando como método a fenomenologia hermenêutica. Participaram no estudo dez enfermeiros (as), sendo sete do sexo feminino e três do sexo masculino. As descrições experienciais foram obtidas através de entrevistas abertas. Os significados essenciais do fenómeno em estudo emergiram com o apoio do programa de análise qualitativa QSRNvivo7. Desta análise destacaram-se quatro temas intimamente interligados: recordando o percurso de aquisição de competência para cuidar em UCI; (re) criando o ambiente de cuidado da UCI; experienciando o cuidado a pessoas idosas na UCI e; valorizando a família no cuidado a pessoas idosas na UCI. A construção da competência para o cuidado a pessoas idosas em UCI percorre a complexidade do ambiente de cuidado e se evidencia num processo evolutivo, dinâmico, simultaneamente individual e colectivo. Requer investimentos constantes que são adquiridos com as situações vividas no quotidiano, com as relações interpessoais e com a formação formal. O ambiente da UCI destaca-se por suas dimensões relacionais e contextuais no processo de cuidado. É caracterizado como um espaço em que o isolamento, a imprevisibilidade, a tensão permanente entre o ser e o não ser e a intensidade do ritmo de trabalho impõem o constante (re) criar e fortalecer das relações interpessoais, do empenho e espírito de colaboração multidisciplinar para a promoção de um ambiente protector e humanizado. O cuidado emerge enquanto processo dinâmico, complexo, diferenciado, criativo, inter-relacional e contextual, centrado na pessoa idosa. Neste processo, a família é valorizada através do acolhimento, orientação e preparação para o encontro com o seu este querido. Os quatro temas evidenciam uma ruptura com a prática tradicional orientada pelo modelo biomédico e com o estereótipo da racionalidade técnica, cujo objectivo é a cura e não o cuidado do ser humano. O cuidado desenvolvido por estes profissionais inclui e transcende a objectividade ao valorizar as emoções, sentimentos, percepções e intersubjectividades que permeiam o viver das pessoas idosas e seus familiares. Embora reconheçam os benefícios e a importância da tecnologia demonstram preocupação em encontrar uma sinergia entre o aperfeiçoamento técnico-científico e um modo mais humano e harmonioso de estar na relação de cuidado. O desvelamento dos significados do cuidado de enfermagem às pessoas idosas numa UCI pode dar visibilidade às suas mais diversificadas características e contribuir para promover abordagens mais humanizadas de cuidado.

ABSTRACT: This study aimed to know the meanings of nursing care for elder people in an intensive care unit (ICU) of a hospital in the central region of Portugal, from the experiences of nurses. It was developed through a qualitative approach, using as a method the hermeneutic phenomenology. Ten nurses participated in the study, seven female and three male. The experiential descriptions were obtained through open interviews. The essential meaning of the phenomenon under study emerged with the support of the program for qualitative analysis QSRNvivo7. From this analysis arose four themes that are intimately related: remembering the route of acquiring the competency to provide care in an ICU; (re) creating an environment of the ICU care; experiencing the caring for elder people in the ICU and; valuing the family in caring for elder people in the ICU. The building of competency for caring elder people in an ICU runs through the complexity of the environment of caring and is evident in an evolutionary, dynamic process, that is simultaneously individual and collective. It requires constant investments that are acquired through the situations experienced in daily life, with the interpersonal relationships and formal training. The ICU environment is highlighted for its relational and contextual dimensions in the caring process. It is characterized as a space in which isolation, unpredictability, permanent tension between the being and the non-being and the intensity of work calls for constant (re) creation and strengthening of inter-personal relations, the commitment and spirit of multidisciplinary collaboration for the promotion of a protective and humanized environment. Caring emerges as a dynamic, complex, differentiated, creative, inter-relational and contextual process centered on the elderly. In this process, the family is valued through the welcoming, orientation and preparation for the encounter with their loved one. The four themes show a break from the traditional practice guided by the biomedical model and the stereotype of a technical rationality whose aim is the cure and not the care of the human being. The caring developed by these professionals includes and transcends objectivity in valuing emotions, sentiments, perceptions and inter-subjectivities that prize the life of the elder people and their families. Although they recognize the benefits and importance of technology, they show concern to find a synergy between the technical-scientific improvement and a more humane and harmonious form in the caring relation. The unveiling of the meanings of nursing caring for elder people in an ICU may provide visibility to its diversified characteristics and contribute to promote a more humanized approaches of caring.
