Cheque-dentista: uma análise de eficiência para Portugal

Abstract

Nos últimos anos, os sistema de vouchers apresenta-se como uma importante ferramenta de combate a desigualdades na prestação de serviços. Grande parte da população continua sem ter acesso a cuidados básicos de saúde devido a problemas socioeconómicos e é nesse contexto que surgiu o Programa Nacional para a Promoção da Saúde Oral (PNPSO) em Portugal. Com o objetivo de combater a cárie dentária, este programa consiste na emissão de cheques-dentista atribuídos aos grupos inseridos no programa. Atualmente surge a necessidade de averiguar o quão eficiente é o comportamento das entidades estatais na prestação de cuidados de saúde e é esta a nossa questão principal. Utilizando o método Data Envelopment Analysis (DEA), analisamos a eficiência técnica das Administrações Regionais de Saúde (ARS), no que respeita ao PNPSO, durante o período 2007-2010. Os resultados do estudo mostram que as alterações estruturais ao PNPSO não resultaram, de uma forma geral, num aumento da eficiência do programa no grupo principal de crianças e jovens. O comportamento de quatro Administrações Regionais foi bastante diferente, não sendo possível concluir que existe realmente uma tendência de aumento de eficiência. Essas diferenças poderão ser explicadas pelo elevado número de habitantes, pelas preferências dos consumidores, pelo preço das consultas entre regiões e, também, pela concentração de médicos dentistas nas várias regiões.

In the past couple of years, the voucher system has presented itself as na important tool to fight the differences when providing services. A great share of the population continues without access to basic health care, due to social economic problems and it is within that context that the Programa Nacional para a Promoção da Saúde Oral (PNPSO) in Portugal arose. With the goal of fighting dental cavities, this program consists in the emission of dentalvouchers to those who are inserted in the program. Nowadays we have the need of knowing how efficient the behavior of the State entities is when providing health care services and that is therefore our main question. Using Data Envelopment Analysis (DEA) system, we are able to analyze the technical efficiency of the regional health administrations (ARS), concerning the PNPSO, during the periods of 2007 to 2010. The results of the study show that the structural changes in the PNPSO didn’t work, in a general way, in an increase of the program’s efficiency regarding the children and teenagers’ group. The behavior of four regional health administrations were quite different from each other, not being possible to determine that there really is a constant tendency in the increase of efficiency. Those differences might be explained by the great number of inhabitants, the consumers’ preferences, the price of the appointments between regions, and also the concentration of dentist doctors in the different regions.
