Abstract: | Em Portugal desconhece-se com precisão a prevalência da Hepatite B, no
entanto, é considerado um país de endemicidade baixa a intermédia. A
influência dos genótipos do Vírus da Hepatite B (VHB) na infecção crónica tem
sido mais intensamente estudada em populações onde a prevalência da
doença é alta. Dos trabalhos publicados com a determinação dos genótipos
em Portugal, poucos caracterizam a população de indivíduos infectados pelo
VHB.
Actualmente existe um grande interesse em identificar os genótipos do VHB
mais prevalentes uma vez que estes podem influenciar na gravidade da
doença, designadamente na evolução para as formas mais graves como
cirrose e o carcinoma hepatocelular, e também na resposta ao tratamento
antivírico, como é sugerido por estudos recentes.
Foi definido como objectivo principal deste trabalho determinar os genótipos do
vírus da Hepatite B nas amostras de doentes com Hepatite B crónica enviadas
para estudo ao Centro Regional de Sangue do Porto pelos hospitais da zona
Norte, que possuem protocolo para estudos analíticos por biologia molecular.
Adicionalmente e como objectivo secundário, determinaram-se também as
mutações que conferem resistência aos fármacos actualmente utilizados na
terapêutica.
Os genótipos foram determinados por um método de sequenciação directa
usando o teste TRUGENE ® HBV Genotyping comercializado pela SIEMENS®.
Os resultados obtidos indicam que o genótipo D é o mais predominante (62%)
seguido do genótipo A (32%). A prevalência de mutações que conferem
resistência aos fármacos foi de 13,4%. Todas as mutações determinadas
conferem resistência à lamivudina sendo também observadas resistências
para o entecavir e adefovir.
A predominância dos genótipos do presente trabalho coincidiu com a
observada em outros estudos idênticos realizados em Portugal bem como com
o está referido para o Sul da Europa ou base mediterrânea, na literatura
consultada. In Portugal, the exact prevalence of Hepatitis B is unknown; however it has a low to medium level of endemicity. The influence of the Hepatitis B virus (HBV) genotypes on chronic infection has been vastly studied in populations with high prevalence. Of all the studies published in Portugal regarding the determination of genotypes, very few focus on the HBV infected population. Nowadays, there is great interest in identifying the most prevalent genotypes of HBV, once these may have an influence on the illness severity, mainly on its evolution towards more severe forms, like cirrhosis and hepatocellular carcinoma, as well as on the response to the antiviral treatment, as suggested in recent studies. The main purpose of this study is to determine the HBV genotypes in chronic HBV patients samples sent to the Centro Regional de Sangue do Porto by hospitals in the North region that have a bio-molecular analytical studies protocol. In addition, it has also been determined the mutations which offer resistance to drugs used in current therapy. The genotypes were determined through a direct sequencing method using the TRUGENE ® HBV Genotyping test developed by SIEMENS. The results show that genotype D is the most predominant (62%), followed by genotype A (32%). The prevalence of mutations that offer resistance to drugs was 13,4%. All determined mutations offer resistance to lamivudina. Resistance to entecavir and adefovir was also been observed. The genotype predominance of this study coincides with that observed in similar studies conducted in Portugal and with what is referred to in the literature for Southern Europe or Mediterranean base. |
