Título: Aplicação de FISH e RQ-PCR no estudo da leucemia mielóide crónica

Resumo: O equilíbrio entre proliferação, diferenciação e renovação das células estaminais encontra-se alterado em pacientes com leucemia mielóide crónica (LMC). Este efeito resulta da actividade contínua da tirosina cinase expressa como consequência da translocação cromossómica recíproca t(9; 22) (q34; q11 ) da qual resulta o cromossoma Filadelfia (Ph). Os recentes avanços de técnicas citogenéticas e moleculares, tais como Fluorescent In Situ Hybridisation (FISH) e a Reverse Transcription Quantitative Polymerase Chain Reaction (RQ-PCR) têm substituído o Western Blotting e o Southern blotting em complemento do estudo do cariótipo. Constituem também boas alternativas em termos de rapidez e eficácia na confirmação do diagnóstico e monitorização da terapêutica. A comparação das duas técnicas é essencial para determinar o valor destas na monitorização e diagnóstico da LMC. Na escolha de um teste laboratorial é ponderada também a necessidade de métodos rápidos e sensíveis para avaliar a resposta à terapêutica de pacientes com LMC. Este estudo tem como como objectivo avaliar a sensibilidade e utilidade clínica das técnicas de RQ-PCR e de FISH no diagnóstico e na monitorização da LMC. O estudo incidiu sobre um grupo de 38 pacientes seguidos nas consultas de Hematologia dos Hospitais da Universidade de Coimbra, aos quais foi diagnosticada LMC e que foram alvo de análises por FISH e RQ-PCR. Através da correlação de Spearman foram avaliadas as concordâncias entre os resultados de FISH e de RQ-PCR. Obteve-se uma correlação significativa (0,79) entre resultados dos dois métodos no que diz respeito à concordância entre resultados positivos e negativos. Foram encontrados resultados concordantes entre FISH e RQ-PCR em 92,1% (35 pacientes) dos casos analisados e uma discordância em 7,9% (3 pacientes). O estudo realizado apoia a possibilidade de diagnosticar e controlar a LMC por meio de FISH e por RQ-PCR, embora seja necessário o alargamento do estudo combinado por FISH e RQ-PCR a um maior número de amostras a fim de proporcionar uma avaliação mais eficaz do diagnóstico e acompanhamento da LMC.
The balance between proliferation, differentiation and renewal of stem cells is altered in patients with chronic myeloid leukemia (CML) due to the persistent activity of tyrosine kinase, resulting from the reciprocal chromosomal translocation t (9; 22) (q34, q11), that originates the so called Philadelphia chromosome (Ph). Recent advances in cytogenetic and molecular techniques such as Fluorescent-In-Situ-Hybridisation (FISH) and Reverse Transcription Quantitative Polymerase Chain Reaction (RQ-PCR) have replaced the Western and Southern Blotting techniques and complement the karyotype anaysis, confuguring a good alternative in terms of time and efficiency in the confirmation of the diagnosis and in the monitoring of the therapy. For the choice of a laboratory test is the need for rapid and sensitive methods to assess response to therapy of patients with CML. A comparison of two techniques is essential to determine the value of such approaches. This study aimed the evaluation of the sensitivity and clinical utility of the technique of RQ-PCR and FISH in the diagnosis and monitoring of CML. The study focused on a group of 38 patients followed in the clinical hematology service of the University Hospitals of Coimbra, which were diagnosed with CML and have also been studied by FISH and RQ-PCR. The agreement of the FISH and RQ-PCR approaches was assessed through the correlation between the results of the two techniques. With regard to the consistency of positive and negative results, a correlation of 0.79 between the two methods was obtained. The results of FISH and RQ-PCR were in agreement in 92.1% (35 patients) of cases analyzed and a discrepant in 7.9% (3 patients). The results of this study supports the potential interest of monitoring CML by FISH and RQ-PCR. Because these techniques have advantages and disadvantages, an enlarged study of the combined use of FISH and RQ-PCR would be valuable in order to provide a more effective tool for the diagnose and monitoring of the treatment of CML.
