Farmacovigilância: as atitudes dos enfermeiros perante a notificação

Abstract

Introdução: Uma Reacção Adversa a Medicamentos (RAM) é uma resposta prejudicial e indesejada a um medicamento. É imprescindível a sua monitorização de modo a garantir a qualidade e a segurança dos medicamentos, devendo ser notificadas ao Sistema Nacional de Farmacovigilância (SNF). Os enfermeiros podem desempenhar um papel fundamental nesta notificação mas, por razões ainda desconhecidas, existe uma elevada taxa de subnotificação por parte destes profissionais. Objectivo: Conhecer e compreender as atitudes e conhecimentos associados à subnotificação de RAM pelos Enfermeiros da Região correspondente à Administração Regional de Saúde do Centro de Portugal. Métodos: O estudo incidiu sobre uma amostra de 30 enfermeiros em hospitais e centros de saúde da região centro de Portugal. A cada profissional de saúde foi entregue um questionário estruturado com as atitudes e conhecimentos dos enfermeiros perante a notificação. Algumas destas atitudes basearam-se nos sete sinais de Inman, que foram medidas utilizando uma escala analógica visual contínua. As respostas foram registadas num intervalo que vai de 0 (totalmente em desacordo) a 10 (totalmente de acordo). O questionário foi entregue a uma amostra de 30 enfermeiros: 20 enfermeiros que exercem as suas funções no Hospital Infante D. Pedro E.P.E (10 no serviço de cirurgia e 10 no serviço de medicina) e 10 enfermeiros que exercem a sua actividade profissional nos Centros de Saúde de Vagos e de Oliveira do Bairro. O mesmo questionário foi distribuído 2 vezes à mesma amostra de enfermeiros num intervalo de 2 semanas. A fiabilidade e a reprodutibilidade do questionário foram determinadas pelo Coeficiente de Correlação Intraclasse. Resultados: Os resultados mostram que há diferenças em termos de conhecimentos e atitudes que condicionam o enfermeiro como notificador. Relativamente aos sete sinais de Inman os que mais interferem são: Incentivo financeiro (ICC 0,77; intervalo de confiança 95% 0,57- 0,88; p 0,0000); A complacência (ICC 0,70, intervalo de confiança 95% 0,46 - 0,85 e p 0,0000; A insegurança (ICC 0,69, intervalo de confiança 95% 0,44 - 0,84 e p 0,0000); A desconfiança (ICC 0,65, intervalo de confiança 95% 0,39 - 0,82 e p 0,0000); A ignorância do sistema de notificação (ICC 0,65, intervalo de confiança 95% 0,38 - 0,82 e p 0,0000); Em relação a outras atitudes, as que mais interferem são as atitudes relacionadas com a falta de tempo para preencher a ficha branca (ICC 0,75, intervalo de confiança 95% 0,54- 0,87 e p 0,0000); as atitudes relacionada com o direito de prescrever do notificador (ICC 0,90, intervalo de confiança 95% 0,79 - 0,95 e p 0,0000). Conclusão: Este estudo demonstra que existem atitudes que influenciam fortemente a subnotificação. Recomendamos campanhas de sensibilização e a implementação de intervenções educacionais baseadas nas atitudes aqui encontradas.

Introduction: An Adverse Drug Reaction (ADR) is a harmful and unintended response to a drug. It is essential its monitoring to ensure the quality and safety of medicines and should be notified to the National Pharmacovigilance System (NPS). Nurses can play a key role in this notification but, for reasons still unknown, there is a high rate of underreporting by these professionals. Objective: To know and understand the attitudes and knowledge towards the notification of RAM between the nurses of the region corresponding to the Regional Health Administration Center of Portugal. Methods: This study focused on a sample of nurses in hospitals and health centers in central Portugal. Each health professional was given a structured questionnaire with the attitudes and knowledge of nurses before the notification. Some of these attitudes were based on the seven signs of Inman, which were measured using a visual analogue scale continuous. The responses were recorded in a range that goes from 0 (completely disagree) to 10 (completely agree). The questionnaire was given to a sample of 30 nurses: 20 nurses who perform their duties in the Hospital Infante D. Pedro EPE (10 in the surgical department and 10 in the service of medicine) and 10 nurses who carry out their work at the Health Center of Vagos and Oliveira do Bairro. The same questionnaire was distributed 2 times the same sample of nurses in a range of 2 weeks. To test the reliability and reproducibility of the questionnaire was determined by the intraclass correlation coefficient. Results: The results show that there are differences in knowledge and attitudes that affect the nurse as a notifier. Of the seven signs of Inman who influence the most are: Financial incentives (ICC 0.77, CI 95% 0.57 - 0.88; p 0.0000); Complacency (ICC 0.70, Cl 95% 0.46 - 0.85 and p 0.0000; Insecurity (ICC 0.69, CI 95% 0.44 - 0.84 and p 0.0000) ; Diffidence (ICC 0.65, CI 95% 0.39 - 0.82 and p 0.0000); Ignorance of the notification system (ICC 0.65, CI 95% 0.38 - 0.82 and p 0.0000). For other attitudes, which influence the most are the attitudes about the lack of time to fill out the white plug (ICC 0.75, CI 95% 0.54 to 0.87 and p 0.0000), attitudes related to the right to prescribe the notifier (ICC 0.90, confidence interval 95% 0.79 to 0.95 and p 0.0000). Conclusion: This study demonstrates that there are attitudes that strongly influence underreporting. We recommend awareness campaigns and implementation of educational interventions, based on these attitudes found here and the continuity of studies including Case-Control Studies.
