Abstract: | Descoberto na década de 70, o antígeno leucocitário humano (HLA) e a sua
relação com as rejeições de órgãos e tecidos transplantados, desde cedo
despertou interesse na comunidade científica. Estas moléculas extremamente
variáveis, agrupam-se em três classes. As classes I e II têm afinidades
estruturais e partilham funções da regulação do sistema imune. As proteínas,
da classe III, assumem diferentes funções e não estão estruturalmente
relacionadas entre si.
O mecanismo de patogénese envolvendo as EAP do HLA-B27, permanece
largamente desconhecido, apesar de se saber que a sua principal função
imunológica é a de ligar peptídeos antigénios e apresentá-los aos linfócitos T.
Porém novas hipóteses estão a ser equacionadas, designadamente a acção
protectora que alguns subtipos alélicos parecem conferir aos seus portadores.
Este trabalho foca o gene HLA-B27 enquanto fator envolvido em doenças
auto-imunes, como as EAP. Entre estas, destaca-se a espondilite
anquilosante, com uma prevalência deste antigénio de cerca de 90% em
indivíduos caucasianos, sendo o sexo masculino cinco vezes mais afectado. A
análise do gene HLA-B27 não é apenas relevante nos indivíduos sintomáticos,
mas também em indivíduos assintomáticos aparentados com portadores da
doença, que apresentem o fenótipo e risco acrescido de desenvolverem
doença. A importância da detecção da molécula da classe I codificada pelo
HLA-B27 o mais precocemente possível relaciona-se com a vantagem de
iniciar a terapêutica adequada o mais breve possível.
O objectivo geral deste trabalho é comparar a abordagem fenotípica
correspondente à análise por citometria de fluxo (CF), com a abordagem
genotípica correspondente à deteção por PCR com o kit GenoQuick, no estudo
de alterações de expressão do gene HLA-B27 e avaliar o valor das duas
abordagens no diagnóstico molecular das espondiloartropatias (EAP).
Os resultados obtidos permitem concluir que o método baseado em PCR
revelou maior sensibilidade, produzindo um maior número de casos positivos
uma vez que a análise por PCR deteta sequências no DNA correspondentes
às várias formas alélicas enquanto que a análise por citometria deteta os
antigénios resultantes da expressão dos genes. Discovered in the 70s, the human leukocyte antigen (HLA) is involved in the rejection of transplanted organs and tissues and since then, it has drew the attention of the scientific community. These molecules are highly variable and can be divided in three classes. The classes I and II are structurally related and share common functions of regulation of the immune system. The class III proteins, have different functions and are not structurally related to each other. The mechanism of pathogenesis involving HLA-B27 in the spondyloarthropathies (SPA) remains largely unknown, although the role of HLA-B27 in connecting peptide antigens and presenting them to T lymphocytes is clearly demostrated. New functions, such as the protective action that some allelic subtypes seem to provide to their carriers, are being explored as newhypotheses. This work focuses on the gene HLA-B27 as a factor involved in autoimmune diseases, such as SPA from which ankylosing spondylitis emerges as a relevant pathology. The prevalence rate of this antigen in caucasian populations is approximately 90 %, being men approximately five times more affected than women. This analysis of HLA-B27 aleles or their expression product is not only relevant in symptomatic individuals, but also in asymptomatic individuals, and especially among the relatives of patients with the disease, because they represent an elevated risk of the disease. It is desirable that the detection of this class I molecule of is achieved at the most early stage possible and that an adequate therapy can be initiated. The objective of this work was to assess the value of the phenotypic analysis by Flow Cytometry (FC), with genotypic analysis by PCR with the kit GenoQuick, in the study of changes in expression of HLA-B27 and in the molecular diagnosis of (SPA). The results show that the genotypic approach based on PCR provides greater sensitivity, producing a larger number of of positive results because it detects sequences in the DNA corresponding to various allelic forms while the cytometric analysis detects the antigenic protein resulting from expression of genes. |
