Abstract: | The treatment of root canals is performed when there is some damage to the
tooth, due to advanced caries or fractures, which end up causing the exposure
of the dental pulp. The pulp, which in a healthy tooth is in a sterile environment,
eventually ends up being exposed to microorganisms present in the oral cavity
that will lead to infection and necrosis. It is known that two of the most prevalent
microorganisms in the endodontic infections are Enterococcus faecalis and
Candida albicans. In order to preserve the tooth, the solution is to perform an
endodontic treatment in an attempt to remove the infected tissues and especially
the microorganisms, by cleaning/disinfecting, applying intracanal medication and
filling and sealing the tooth. But due to the increased resistance of these
microorganisms to the intracanal medications currently used, and the fact that
the tooth has a complex anatomical structure, sometimes the
cleaning/disinfection of the tooth and the application of intracanal medication are
not enough effective in removing the microorganisms from inside the tooth. In
this way, this master's thesis proposes a new approach for the development of
an intracanal medication, and an irrigant, using new bioactive glass compositions
and the crude extract from the marine fungus Penicillium lusitanum, respectively.
Bioactive glasses have been shown to possess properties of great interest for
this type of treatment, such as the capacity of bone formation, dentine
regeneration and of exerting antimicrobial activity. Thus, the bioactive glass
samples produced in this work were morphologically, physically and biologically
characterized. In the case of marine fungi, it is known that they have the ability
to produce complex chemical molecules of great interest and little explored.
Previous studies with this marine fungus, the Penicillium lusitanum, confirmed
the capacity of its extracts to inhibit the growth of some microorganisms, such as
clinical isolates of Candida albicans. Through the assays of antimicrobial activity
carried out in this work, it was possible to observe that bioactive glass samples,
at a concentration of 5 and 15 mg mLˉ¹, exert antimicrobial activity against both
microorganisms tested, E. faecalis and C. albicans. The same was not observed
with P. lusitanum extracts, at a concentration of 10 mg mLˉ¹. However, there is
still a need for further tests in order to clearly understand all the potential for this
type of application. O tratamento dos canais radiculares é realizado quando ocorre alguma lesão no dente, devido a cáries avançadas ou fraturas, que acabam por originar a exposição da polpa dentária. A polpa, que num dente saudável se encontrava num ambiente estéril, acaba por ficar exposta a microrganismos presentes na cavidade oral que vão originar a infeção e necrose. Sabe-se que dois dos microrganismos mais comummente presentes nas infeções endodônticas são o Enterococcus faecalis e a Candida albicans. De forma a preservar o dente, a solução passa então por efetuar este tratamento endodôntico de forma a tentar remover os tecidos infetados, e os microrganismos, através da limpeza/desinfeção, aplicação de uma medicação intracanalar e a realização do preenchimento e selagem do dente. Mas devido ao aumento da resistência destes microrganismos às medicações intracanalares atualmente utilizadas, e ao facto de o dente possuir uma estrutura anatómica complexa, por vezes a limpeza/desinfeção do dente e a aplicação de medicação intracanalar não são suficientemente eficazes na remoção dos microrganismos do interior do dente. Desta forma, esta tese de mestrado propõe uma nova abordagem para o desenvolvimento de uma medicação intracanalar, e de um irrigante, através da utilização de novas composições de vidros bioativos, e do extrato bruto do fungo marinho Penicillium lusitanum, respetivamente. Os vidros bioativos têm vindo a demonstrar possuir propriedades de grande interesse para este tipo de tratamento, tais como, capacidade de regeneração e formação óssea, capacidade de regenerar a dentina e de exercer uma atividade antimicrobiana. As amostras de vidro bioativo produzidas neste trabalho foram morfologicamente, fisicamente e biologicamente caracterizadas. No caso dos fungos marinhos, sabe-se que têm a capacidade de produzir moléculas químicas complexas de grande interesse e pouco exploradas. Em estudos já realizados com o fungo marinho, Penicillium lusitanum, foi possível verificar a capacidade dos seus extratos em inibir o crescimento de alguns microrganismos, como isolados clínicos de Candida albicans. Através dos ensaios de atividade antimicrobiana realizados neste trabalho, foi possível observar que existe nas amostras de vidro bioativo, numa concentração de 5 e 15 mg mLˉ¹, um potencial antimicrobiano contra ambos os microrganismos testados, E. faecalis e C. albicans. Já o mesmo não se observou com os extratos do P. lusitanum, numa concentração de 10 mg mLˉ¹. No entanto, ainda existe a necessidade de realizar mais testes de forma a ser possível compreender claramente todo o potencial para este tipo de aplicação. |
