Abstract: | Enquadramento: O objetivo dos tratamentos de radioterapia é irradiar o
volume alvo minimizando a dose recebida pelos órgãos de risco adjacentes
(OAR’s). Uma correta colimação do feixe de radiação ao volume alvo
utilizando o colimador multilâminas (MLC) permite reduzir a dose nos OAR’s
para valores abaixo dos valores de tolerância impostos pela radiobiologia
tecidular. O MLC consiste em vários pares de folhas que se movem de forma
independente e que permitem bloquear a radiação do feixe incidente.
Diferentes MLCs, com diferentes características, permitem executar planos
de tratamento e administração de doses de forma diferente. Objetivos:
Comparar as características de dois colimadores multilâminas e avaliar as
vantagens dosimétricas da utilização de cada um, no planeamento de
tratamentos de Radiocirurgia, consoante a patologia, forma e volume do
tumor. Métodos: Foram estudados 30 casos de radiocirurgia cujo plano foi
executado com o MLC Agility™ no sistema de planeamento (TPS) Monaco®,
utilizando o algoritmo Monte Carlo para o cálculo de planimetrias com a
técnica de Volumetric Modulated Arc Therapy (VMAT). Utilizando planos já
executados com MLC Agility™ foi feita a sua reotimização, mantendo a
configuração de campos, as mesmas funções objetivo de otimização, mas
com o MLC Apex™. Resultados: Os casos clínicos foram tratados com uma
dose total entre 12 e 20 Gy numa única fração e 80% (n=24) das lesões
eram irregulares com um volume tumoral que variava entre 0,06 e 15,21 cm3
(mediana= 2,68 cm3
). Para os 30 planos de SRS VMAT, ambos os
colimadores apresentaram resultados semelhantes em termos de cobertura
do PTV clinicamente aceitável para o tratamento de SRS (p= 0.84, n= 60).
Não se observaram diferenças significativas nos índices da qualidade,
exceto no índice de gradiente, onde se observa uma melhoria
estatisticamente significativa com o micro-MLC ApexTM, mostrando alguma
vantagem clínica nos neurinomas e alguns meningiomas. Conclusão: O
tratamento de SRS com o micro-MLC Apex™ apresentou resultados
dosimétricos muito semelhantes ao MLC Agility™, em termos de cobertura
do PTV, índices da qualidade do plano e limites de dose a cumprir nos
OAR’s. Não houve diferenças estatisticamente significativas nos índices da
qualidade, exceto no índice de gradiente, em que se observa uma melhoria
com o micro-MLC Apex™, que nos permite concluir que terá uma vantagem
clínica mais significativa nos neurinomas devido ao menor tamanho desses
tumores. Background: The goal of radiotherapy treatments is to irradiate the target volume minimizing the dose received by the adjacent organs at risk (OAR’s). Correct beam collimation to the target volume using the multi-leaf collimator (MLC) reduces the dose in OAR’s below the tolerance values imposed by tissue radiobiology. The MLC consists of several independently moving leaf pairs that block the incident radiation beam. Different MLCs, with distinct characteristics, allow the execution of treatment plans and administration of different doses. Purpose: Comparison of the characteristics of two multi-leaf collimators and evaluation of the dosimetric advantages of their use in the planning of radiosurgery treatments, according to the pathology, shape and volume of the tumour. Methods: Thirty cases of radiosurgery whose planning was performed with MLC Agility™ in the Monaco® planning system (TPS), using the Monte Carlo algorithm for the calculation of planimetry using the technique Volumetric Modulated Arc Therapy (VMAT), were studied. Plans will be reoptimized, maintaining field configuration, and the same objectives and optimization functions, but with MLC Apex™. Results: The clinical cases were treated with a prescription dose of 12 to 20 Gy in a single fraction and 80% (n=24) of the lesions included in the initial review were irregular, with gross tumour volume ranging from 0.06 to 15.21 cm3 (median: 2.68 cm3 ). For the 30 VMAT SRS plans, both collimators showed comparable results in terms of clinical acceptable PTV coverage objective for SRS treatment (p= 0.84, n=60). There was no significant difference in the quality metrics except in the gradient index, where a statistically significant improvement is observed with the micro-MLC ApexTM, showing some clinical advantage in neurinomas and some meningiomas. Conclusion: The treatment of the SRS with the micro-MLC Apex™ shows results very similar to the MLC Agility™, in terms of PTV coverage, plan quality metrics and dose limits the sparing of adjacent organs. There were no statistically significant differences in the quality metrics, except in the gradient index, in which an improvement is observed with the micro-MLC Apex™, which allows us to conclude that it will have a more significant clinical advantage in neurinomas due to the smaller size of these tumors. |
