Cuidados em serviço de apoio domiciliário

Abstract

A tese intitulada Cuidados no Serviço de Apoio Domiciliário reúne quatro estudos. O 1º apresenta um estudo de caso realizado em duas instituições de Braga. O 2º artigo caracteriza o perfil de funcionamento do SAD existente. O 3º artigo que estuda as variáveis associadas com os indicadores de qualidade do serviço de apoio domiciliário português. O 4º define o perfil de idosos caraterizados pelo RAI-Home Care nestas 20 IPSS. Como principais instrumentos utilizaram-se o protocolo de avaliação em Serviço de Apoio Domiciliário e o Instrumento de Classificação RAI-Home Care. Na realização dos estudos, a partir de um método misto quantitativo e qualitativo, no 1º estudo optou-se por uma metodologia qualitativa, com a aplicação de 11 entrevistas semiestruturadas. No 2º estudo, a aplicação do protocolo de avaliação por inquéritos por questionário e o recurso à estatística descritiva e às inferências não paramétricas, no 3º estudo, as odds ratios - razão de chances ou razão de possibilidades e, no 4º estudo através da aplicação do RAI-Home Care utilizou-se o recurso à matriz de dados pelas árvores de classificação por perfil e modelos logísticos univariáveis. Como principais resultados as instituições o SAD tem um perfil de funcionamento tradicional e assistencial na realização da prestação dos serviços de forma universal e não individual por necessidades de perfis encontrados. Através dos perfil de funcionamento do SAD encontrado e os indicadores de qualidade existentes nas IPSS e os perfis de idosos encontrados, conclui-se que o SAD está a mudar, sendo que foram apresentadas novas soluções, que passam por reconduzir os planos de cuidados, com a introdução de novas formas de financiamento, novos instrumentos de avaliação, novos serviços, e a definição de perfis de classificação de utentes, com a introdução de criação de critérios de elegibilidade e implicações ao nível do financiamento.

The thesis entitled Care in the Home Support Service gathers four studies. The first presents a case study carried out in two institutions in Braga. The 2nd article characterises the existing SAD's operating profile. The 3rd article studies the variables associated with the quality indicators of the Portuguese home care service. The 4th article defines the profile of elderly people characterized by RAI-Home Care in these 20 IPSS. The main instruments used were the assessment protocol in the Home Support Service and the RAI-Home Care Classification Instrument. In the first study, a qualitative methodology was used, with 11 semi-structured interviews. In the 2nd study, the application of the evaluation protocol by questionnaire surveys and the use of descriptive statistics and nonparametric inferences, in the 3rd study the odds ratios - odds ratio or odds ratio of possibilities, and in the 4th study through the application of the RAI-Home Care we used the use of the data matrix by the profile classification trees and univariable logistic models. As main results the institutions the SAD has a traditional and assistencial profile in the accomplishment of the provision of the services in a universal way and not individual for needs of profiles found. Through the functioning profile of the SAD found and the existing indicators of quality in the ONG´S and the profiles of the elderly found, it is concluded that the SAD is changing, with new solutions being presented, which include the renewal of care plans, with the introduction of new forms of financing, new evaluation instruments, new services, and the definition of user classification profiles, with the introduction of the creation of eligibility criteria and implications at the level of financing.
