dc.description.abstractIntrodução: A Síndrome Mielodisplásica (SMD) é um grupo heterogéneo de neoplasias clonais da medula óssea. Embora extremamente complexa, a sua fisiopatologia envolve inúmeros mecanismos genéticos, epigenéticos e mediados pelo sistema imune que levam a uma falha no equilíbrio normal entre a proliferação celular, diferenciação celular e apoptose. É caraterizada por uma hematopoiese ineficaz com alterações na proliferação e diferenciação mieloide das células estaminais hematopoiéticas. Manifesta-se por uma displasia morfológica nas células de diferentes linhas hematopoiéticas, por uma medula óssea normalmente hipercelular e por citopenias periféricas de gravidade variável e que podem progredir para uma leucemia mieloide aguda. As anomalias citogenéticas estão presentes em cerca de 50% dos casos de SMD primária, que incluem: a deleção do braço longo do cromossoma 5 (5q-), deleção do braço longo do cromossoma 7 e monossomia do 7 (-7\7q-), trissomia do 8, deleção do braço longo do cromossoma 20 (20q-) e ausência do cromossoma Y(-Y). Objetivos: Avaliar a presença das alterações cromossómicas acima descritas nas células CD34+ (blastos) purificadas, bem como, nos eritroblastos, células de linha a neutrófilo e monocítica, permitindo perceber melhor o tamanho do clone SMD em cada linha e se este se relacionava com a maior ou menor severidade das citopenias; se uma maior percentagem de células com uma alteração genética ocorria nos subtipos mais graves de SMD (EB1 e EB2) e qual/quais as alterações genéticas que afetavam uma maior percentagem de células. Devido à maior sensibilidade desta estratégia, foi também um objetivo desta tese, identificar quais as alterações genéticas primárias e secundárias em casos com um cariótipo complexo. Metodologia: O grupo de estudo foi constituído por amostras de medula óssea de 30 doentes com diagnóstico de SMD e com pelo menos uma das alterações genéticas acima mencionadas, dos 203 casos estudados e com suspeita de SMD. Pesquisou-se a existência de alterações genéticas por hibridização in situ, por fluorescência (FISH) em diferentes populações celulares purificadas (células CD34+, eritroblastos, monócitos e neutrófilos) por FACS. Resultados: Nos 203 casos com suspeita de SMD, 30 (15%) dos casos apresentavam pelo menos uma alteração cromossómica. Observou-se uma maior percentagem de células CD34+ e de células das diferentes linhas hematopoiéticas estudadas onde se observou a presença de uma determinada alteração genética no subgrupo SMD-EB2. De acordo com os resultados obtidos neste trabalho, apenas 5 dos 30 casos analisados apresentavam mais do que uma alteração genética. Nestes, e com base na percentagem de células afetadas, os resultados sugerem que a 5q-, 7q- e -Y parecem corresponder a alterações genéticas primárias e a 20q- e +8 a secundárias. Neste estudo não se observou qualquer relação entre a percentagem de células afetadas (com uma alteração genética) de uma determinada linha e o grau da citopenia. Conclusão: Demonstrou-se a importância da realização do estudo das alterações genéticas por FISH principalmente em células CD34+ purificadas, permitindo obter uma maior sensibilidade na deteção dessas mesmas alterações genéticas, ter uma ideia do tamanho do clone SMD nessas células e da hematopoiese displásica e, por outro lado, perceber quais foram as alterações genéticas primária e secundárias em casos de cariótipo complexo.pt_PT
dc.description.abstractIntroduction: Myelodysplastic Syndrome (MDS) is a heterogeneous group of clonal neoplasms of the bone marrow. Although extremely complex, the pathophysiology involves numerous genetic mechanisms, epigenetic and mediated by the immune system that lead to a failure in the normal balance between cell proliferation, cell differentiation and apoptosis. It is characterized by ineffective hematopoiesis with changes in myeloid proliferation and differentiation of hematopoietic stem cells. It is manifested by a morphological dysplasia in cells of different hematopoietic lineages, by a hypercellular bone marrow and by peripheral cytopenias of varying severity and which may transform into acute myeloid leukemia. Cytogenetic abnormalities are present in about 50% of primary MDS cases including: deletion of the long arm of chromosome 5 (5q-), deletion of the long arm of chromosome 7 and monosomy of 7 (-7 \ 7q-), trisomy 8, deletion of the long arm of chromosome 20 (20q-) and absence of the Y chromosome (-Y). Aims: To evaluate the presence of the chromosomal abnormalities described above in the purified CD34 + cells (blasts), as well as in the erythroblasts, neutrophil and monocytic cells allowing a better understanding of the size of the SMD clone in each lineage and its relation to the it was related to the degree of cytopenias; if a higher percentage of cells with genetic changes occurred in the more severe subtypes of MDS (EB1 and EB2) and which genetic abnormalities affect a higher percentage of cells. Due to the greater sensitivity of this strategy, it was also a goal of this thesis, to identify the primary and secondary genetic alterations in cases with a complex karyotype. Methodology: The study group consisted of bone marrow samples from 30 patients diagnosed with MDS and at least one of the aforementioned genetic alterations from the 203 cases studied and with suspected MDS. We investigated the existence of genetic alterations by fluorescence in situ hybridization (FISH) in different purified cell populations (CD34 + cells, erythroblasts, monocytes and neutrophils) by FACS. Results: In 203 cases with suspected MDS, 30 (15%) of the cases had at least one chromosomal abnormality. A higher percentage of CD34 + cells and cells of the different hematopoietic lines studied were observed when the presence of a certain genetic alteration in the subgroup SMD-EB2 was observed. According to the results obtained, only 5 of the 30 cases analyzed presented more than one genetic alteration. In these, and based on the percentage of affected cells, the results suggest that 5q-, 7q- and -Y appear to correspond to primary and 20q- and +8 to secondary genetic alterations. In this study, no relationship was observed between the percentage of affected cells (with a genetic alteration) of a given line and the degree of cytopenia. Conclusion: Our results show, on one hand, the importance of investigating genetic alterations by FISH in purified CD34 + cells, allowing a greater sensitivity in the detection of these genetic alterations, evaluating the size of the SMD clone in these cells and the dysplastic hematopoiesis and, on the other hand, to analyze which the primary and secondary genetic alterations were found in cases of complex karyotype.pt_PT
