Análise do workflow imagiológico em contexto de urgência no âmbito da via verde AVC

Abstract

Dada a necessidade de garantir a celeridade na prestação de cuidados no âmbito dos utentes com um quadro clínico indicativo de Acidente Vascular Cerebral (AVC) em fase aguda, o presente trabalho tem por objetivo analisar e identificar, através da modelação dos processos de requisição e realização de estudos imagiológicos, fatores, procedimentos e rotinas que possam condicionar a prestação de cuidados a utentes no contexto da Via Verde de AVC (VV AVC). Numa primeira fase deste projeto, realizou-se o mapeamento do workflow e dataflow imagiológico instituído numa unidade hospitalar, seguindo-se a caracterização dos Sistemas de Informação (SI) utilizados pelos profissionais de saúde, através de resposta anónima a questionários, terminando com a análise dos tempos associados aos processos de realização de Tomografia Computorizada Crânio-Encefálica (TC CE) aos utentes com suspeita de AVC em fase aguda, através da informação registada nos SI utilizados. Do total de 464 utentes que realizaram TC CE no contexto em estudo, 198 realizaram também Angio TC. Analisando os tempos associados aos processos de Diagnóstico por TC CE, verificamos que este demora em média 47 minutos. Quando analisamos o tempo associado ao relatório da TC CE, verificamos que os estudos demoram em média 24 minutos a ser relatados. Já no processo que engloba a realização de TC CE, Angio TC e Pós-Processamento, concluímos que, para uma amostra de 198 utentes, o processo demora em média 26 minutos. No entanto, verificou-se uma variação significativa nos tempos despendidos nos processos e subprocessos que suportam a execução de estudos de TC CE. O trabalho desenvolvido permitiu a identificação de situações que podem resultar em pontos de melhoria na prestação de cuidados no âmbito do diagnóstico e caracterização do AVC dos utentes no contexto VV AVC, as quais podem ser alvo de trabalhos futuros

Given the need to guarantee a prompt response in the context of patients with an acute clinical manifestation of Stroke, this study aims to analyse and identify, through the modelling of the requisition processes and the accomplishment of studies, imaging studies, factors, procedures and routines that can condition the provision of care to the users in the context of stroke (VV AVC). In a first phase of this project, the mapping of the workflow and imaging dataflow instituted in the hospital unit was carried out, followed by the characterization of the Information Systems (IS) used by the health professionals, through anonymous response to questionnaires, ending with the analysis of the time associated with the processes of performing head Computed Tomography (Head CT) studies in patients with suspected acute stroke, through the information recorded in the IS used. Of the total of 464 patients who underwent Head CT, 198 also performed CT Angiography. Concerning the Head CT diagnosis process, this takes an average of 47 minutes. However, 50% of the patients in the sample get the diagnosis within 30 minutes. When we analyse the time associated with the Head CT report, we find that the studies take an average of 24 minutes to be reported. When we analyse the process involving Head CT, CT Angiography and Post- Processing, we conclude that for a sample of 198 users, the process takes an average of 26 minutes. However, there was a significant variation in the time spent in the processes and subprocesses that support the execution of CT studies. The work developed allowed the identification of situations that may result in improvement points in the provision of health care in the context of the diagnosis and characterization of the Stroke in patients in the VV stroke context, which may be the subject of future work
