dc.description.abstractEste trabalho visa avaliar o uso das hastes no fémur quando existem defeitos ósseos significativos neste que possam influenciar o comportamento biomecânico da tíbia. Para tal, foi seleccionada a prótese de joelho P.F.C. Sigma Knee System como elemento para a realização do estudo comparativo. Realizaram-se três modelos experimentais e numéricos, nomeadamente a prótese com haste cimentada, outra com haste press-fit e um modelo sem haste. No modelo experimental começou-se por seccionar o fémur e a tíbia, fixaramse a suportes e colocaram-se no dispositivo utilizado para a realização de ensaios. Colocou-se extensómetros na tíbia, aplicou-se uma força estática de 2100N e mediu-se as deformações. Os desvios observados entre os modelos demonstraram potencialização de stress-shielding na zona posterior da tíbia com haste press-fit. O estudo numérico teve como objectivo reproduzir os modelos o mais próximo da realidade, colocando-se as hastes no fémur com um ângulo de 7º e as restrições de contacto. Comparou-se as deformações obtidas nos modelos numéricos com as extensões dos modelos experimentais, obtendo-se uma boa correlação entre os valores, sendo a correlação mais próxima de 1 obtida no modelo sem haste, em parte devido às simplificações inerentes ao modelo numérico. No sentido de completar os resultados experimentais analisou-se as tensões principais mínimas sofridas no osso esponjoso no modelo numérico, constatando-se que a haste press-fit é a haste que maiores tensões provoca no osso-esponjoso tibial, seguido da haste cimentada. A utilização das hastes modifica a distribuição das tensões no osso esponjoso na interface ossocimento. Com base nos resultados observados, a opção clínica pela haste femoral press-fit deverá ser acautelada, já que esta é a que mais altera o comportamento biomecânico da tíbia na zona proximal. ABSTRACT: The main objective of this thesis was the evaluation of femur stems considering significant bone defects and how those defects influence the biomechanical behavior of the tibia. Finite element and experimental studies were performed concerning the use of stems in revision knee arthroplasty. The P. F. C. Sigma Knee System prosthesis was used within the study. Three numerical and experimental models were done, one model with femur cemented stem, another with press-fit stem and finally one with femur without stem. During the experimental procedure the femur and tibia were sliced, fixed to supports and placed on appropriate assays device. After the strain gauges were placed in the tibia, a static force of 2100 N was applied and the deformation measured. The observed deviation between the models demonstrated potential stress-shielding with press fit stem in the posterior zone of the tibia. In order to simulate the real conditions in the numerical study, the femoral stems were placed with a 7º angle and the contact restrictions established. The numerical deformations were compared with the experimental extensions and a good correlation was obtained between the values. The model without stem was the one that achieved the better correlation might be to the simplifications inherent to numerical model. In order to complete the experimental results, the minimal principal tensions were analyzed in the cancelous bone with the numerical model. The press fit stem was the one that induces more tension in the cancelous bone, followed by the cemented stem. The utilization of femoral stems changes the distribution of tensions in the cancelous bone at the interface bone-cement. Based on these results, the femoral press fit stem is the one that more change the biomechanical behavior of tibia in the proximal zone, so the clinical option for their use must be careful.por
