Abstract: | A articulação do tornozelo é constituída pelas extremidades da
tíbia, da fíbula, e pela superfície superior do tálus. Uma lesão
osteocondral do tálus caracteriza-se por uma lesão no osso e na
cartilagem que o reveste, pondo em causa a integridade da
mesma. Devido à baixa taxa de incidência desta patologia
existem poucos trabalhos dedicados ao estudo da mesma.
O objetivo deste trabalho é alargar o conhecimento acerca desta
lesão. Para a obtenção dos modelos foram utilizadas imagens
provenientes de uma TAC do tornozelo de um paciente, as quais
foram reconstruídas e convertidas para CAD, e posteriormente
adaptadas aos modelos experimentais e numéricos.
Inicialmente foi obtido um modelo físico através de prototipagem
rápida que foi sujeito a ensaios de compressão para a obtenção
de deformações principais. Seguidamente foram efetuados
estudos utilizando os modelos numéricos da articulação e
recorrendo ao método dos elementos finitos. As diferentes
características do modelo, tais como: a dimensão do dano, a
posição da articulação, a geometria da lesão, a regularidade da
sua fronteira e espessura da cartilagem; foram alterados com
vista a analisar a sua influência nas tensões registadas na
cartilagem do tálus. Os valores de tensão obtidos para as
diferentes lesões foram comparados com os valores obtidos para
a cartilagem saudável, permitindo tirar conclusões acerca da sua
influência. Todos estes fatores foram também isolados e
comparados entre si, permitindo perceber quais as combinações
mais críticas para a integridade da cartilagem.
Os resultados obtidos indicam que o fator que mais contribui para
o aumento da tensão na cartilagem do tálus é a dimensão do
dano, registando-se aumentos de 200% da cartilagem saudável
para uma lesão de 10mm, verifica-se também que a existência de
pequenos danos é responsável por um aumento de tensões
equiparável a uma lesão maior (10mm) e também por uma
concentração de tensões em torno do mesmo. Verifica-se que a
posição de flexão plantar do tornozelo em 15º é, entre as
estudadas, a mais crítica. Um fator que causa influência nas
tensões registadas é a irregularidade da periferia do dano, que
poderá causar um maior número de picos de tensão, assim como
aumentos de 75% nas tensões em algumas zonas.
Foi obtida correlação entre o modelo numérico e o modelo
experimental. The ankle joint consists of the extremities of the tibia, fibula and the superior surface of the talus. When there is an osteochondral lesion of the talus, it means that there is damage in the bone and in the cartilage that covers it, compromising the integrity of the joint. Due to the low incidence of this pathology there are very few studies about this topic. The objective of this thesis is to widen the knowledge about this subject. To obtain the models, it was used CT scan images of an injured ankle joint of a patient, these images were reconstructed and converted to CAD, and then adapted to the experimental and numerical models. Initially it was obtained a physical model using rapid tooling methods, this model was subjected to compression tests to determine the principal strains. Then studies were developed by means of numerical models of the ankle joint using the finite elements method. The different characteristics of the model, such as: the dimension of the lesion, the position of the ankle joint, the geometry of the lesion, the regularity of the periphery of the damage and the thickness of the cartilage, were changed with the objective of analyzing their influence in the stress obtained in the talar cartilage. The results were compared between themselves and the healthy cartilage, with the objective of determining the most critical combinations of factors for the integrity of the cartilage. The results obtained indicate that the most critical factor is the size of the lesion, since a lesion of 10mm increases the stress on the cartilage by about 280%, creating the concentration of stress around the damage, it’s also possible to conclude that even the existence of a small damage is responsible to increase the stress near the damage, in the same amount of a 10mm damage. The most critical position of the ankle joint that was studied is the plantar flexion of 15º. The irregularity of the periphery of the damage is responsible for the increase in 75% in some spots, as well as the number of stress peaks. The correlation between numerical and experimental data was obtained. |
