Title: Estudo comparativo: distress emocional, qualidade de vida e otimismo no cancro da mama avançado e na sobrevivência

Abstract: O cancro da mama é o segundo tipo de cancro mais comum, representando 10.9% de todos os casos de cancro diagnosticados mundialmente e ocupa a quinta posição como causa de morte. Este tipo de neoplasia maligna é também a mais frequente entre as mulheres em países desenvolvidos e a principal causa de morte por cancro em todo o mundo. Em Portugal, é também o tipo de cancro com maior taxa de incidência nas mulheres, sendo a principal causa de morte por neoplasia no sexo feminino. O presente estudo tem como objetivo investigar as reações psicossociais que podem estar presentes nas mulheres com cancro da mama, em estadio avançado da doença, nomeadamente o distress emocional, a qualidade de vida e o otimismo e comparar com as mulheres sobreviventes de cancro da mama. A amostra foi constituída por 40 mulheres, distribuídas por dois grupos distintos, 20 sobreviventes de cancro de mama e 20 em estadio avançado. Os instrumentos utilizados foram os seguintes: um Questionário Sociodemográfico e Clínico; a Hospital Anxiety Depression Scale (HADS); o Patient Health Questionnaire-9 (PHQ-9); o European Organization for Research and Treatment of Cancer Quality of Life Questionnaire Core-30 (EORTC QLQ C-30); e o Life Orientation Test Revised (LOT-R). Os resultados indicaram que não existem diferenças significativas nos dois grupos ao nível da sintomatologia depressiva e ansiosa, medida pela HADS. Ao nível do PHQ-9, as mulheres com cancro da mama avançado apresentaram significativamente menos sintomas depressivos do que as mulheres sobreviventes de cancro da mama. No que concerne à QdV, as mulheres com cancro da mama avançado apresentaram diferenças estatisticamente significativas, com melhor QdV nas subescalas funcionamento emocional e social e pior QdV na subescala funcionamento físico do que as sobreviventes. Relativamente ao otimismo, as mulheres com cancro da mama avançado apresentaram um otimismo significativamente mais elevado do que sobreviventes. Este trabalho vem contribuir para diminuir uma lacuna de investigação na área do cancro avançado, que tem vindo a ser identificada a nível nacional e internacional. Particularmente em Portugal, este trabalho é ainda mais relevante, pois que seja do nosso conhecimento não se têm realizado estudos empíricos que investiguem esta população. São indicadas conclusões e implicações clinicas e de investigação, com base nos resultados e no confronto com a revisão da literatura efetuada.
Breast cancer is the second most common type of cancer, representing 10.9% of all cases of cancer diagnosed worldwide and occupies the fifth position as cause of death. This type of malignant neoplasia is also the most frequent among women in developed countries and is the leading cause of cancer death in the world. In Portugal, it is also the type of cancer with the higher rate of incidence in women, being the main cause of death by neoplasia in females. The aim of the present study was to investigate the psychosocial reactions that may be present in women with breast cancer, in advanced stage of the disease, particularly the emotional distress, quality of life and the optimism and compare with women survivors of breast cancer. The sample was composed of 40 women, distributed by two distinct groups, 20 survivors of breast cancer and 20 in advanced stage. The instruments used were the following: Sociodemographic and Clinical Questionnaire; Hospital Anxiety Depression Scale (HADS); Patient Health Questionnaire-9 (PHQ-9); European Organization for Research and Treatment of Cancer Quality of Life Questionnaire Core-30 (EORTC QLQ C-30); Life Orientation Test Revised (LOT-R). The results indicated that there are no significant differences between the two groups in terms of depressive and anxious symptomatology, measured by HADS. In terms of PHQ-9, the women with advanced breast cancer had significantly less depressive symptoms than women survivors of breast cancer. In regards to QoL, women with advanced breast cancer presented significant statistic differences, with better QoL sub-scales of emotional and social functioning and worse QoL sub-scales of physical functioning than the survivors. In terms of optimism, women with advanced breast cancer had a significant higher optimism than survivors. This work will contribute to decrease a gap in research in the area of advanced cancer, which has been identified at national and international levels. Particularly in Portugal, this work is even more relevant, because to our knowledge there has not been performed empirical studies that investigate the population. There are indicated conclusions and clinical and research implications, on the basis of the results and in the comfort of the literature review performed.
