Abstract: | O presente estudo teve como principais objetivos analisar, em
contexto médico-legal, os comportamentos de simulação através do
SIMS (Inventário Estruturado de Simulação de Sintomas; Smith &
Burger, 1997 cit in Simões, 2010; Widows & Smith, 2005), bem como
a relação entre psicopatologia, desejabilidade social e perceção de
ser ou não saudável (física e psicológica) nos comportamentos de
simulação. Também se pretendeu propor um novo ponto de corte
para o SIMS. Para tal foi recolhida uma amostra total de 233 sujeitos,
53 indivíduos no âmbito médico-legal e 180 da comunidade. A
amostra médico-legal, foi subdividida em dois subgrupos com base
nos processos legais dos examinados: (a) Responsabilidades
Parentais e (b) Outras Avaliações. Para este estudo, o protocolo de
avaliação incluiu ainda provas psicométricas como as Matrizes
Progressivas Estandardizadas de Raven (MPR; Raven & Court,
1996; versão portuguesa Infoteste; Raven, 1999), o Rey 15-Item
Memory Test (15-RMT; Rey, 1958; Simões et al., 2010), o Inventário
de Sintomas Psicopatológicos (BSI; Derrogatis, 1982; Canavarro,
1999) e a Escala de Desejabilidade Social de Marlowe-Crowne
(MCSDS; Marlowe & Crowne, 1960; Almiro, Simões, & Sousa, 2012;
Scagliusi et al., 2004). Os resultados demonstraram que no que
concerne aos dois subgrupos amostrais foram observadas diferenças
estatisticamente significativas (pontuações mais altas no subgrupo
Outras Avaliações). Os resultados sugerem também um ponto de
corte de 17 para a dimensão total do SIMS, que pode melhorar a sua
utilidade como instrumento de triagem em contexto médico-legal.
Verificou-se ainda existência de relações positivas e significativas
entre SIMS e BSI. Do mesmo modo, verificou-se que a
desejabilidade social não influencia os resultados do SIMS. Foi ainda
estudada a relação entre pontuações no SIMS e a "perceção de
saúde" do indivíduo sobre si. Os resultados discriminaram
significativamente os indivíduos, obtendo os sujeitos que não se
consideravam saudáveis pontuações mais elevadas no SIMS. Não
foram identificados impactos significativos das variáveis
sociodemográficas (idade, género e escolaridade) no resultado total
do SIMS. As nossas descobertas providenciam suporte para a
utilização do SIMS em âmbito médico-legal. Implicações médico-legais
são discutidas e são apresentadas recomendações para
futuros estudos. The present study had as main objective to analyze, in medical-legal context, malingering by SIMS (Strutured Inventory of Malingered Symptomatology; Smith & Burger, 1997 cit in Simões, 2010; Widows & Smith, 2005) and the relationship between psychopathology, social desirability and perception of whether or not healthy (physical and psychological) with malingering. It is also intended to propose a new cut-score for SIMS. This study had as taken in a total sample of 233 subjects, 53 subjects in the medical-legal context and 180 off the community. Two medico-legal samples were examined, based on the legal processes of the ones examined: (a) Child Custody and (b) Others Assessments. For this study, the assessment protocol also included psychometric tests such as the Raven's Progressive Matrices Standardized (MPR; Raven & Court, 1996; portuguese version Infoteste; Raven, 1999), the Rey 15-Item Memory Test (15- RMT; Rey, 1958; Simões et al., 2010), Brief Symptom Inventory (BSI; Derrogatis, 1982; Canavarro, 1999) and the Marlowe Crowne Social Desirability Scale (MCSDS; Marlowe & Crowne, 1960; Almiro et al., 2012; Scagliusi et al., 2004). The results showed that regarding the two sample subgroups, statistically significant differences were observed (higher scores in the subgroup Other Assessments). The results also suggest a cut-score of 17 for the total score of SIMS, which may improve their utility in screening for malingering among medico-legal context. It was found that there are significant and positive relationships between SIMS and BSI. Similarly, it was found that social desirability does not influence the results of SIMS. It was also considered the relationship between scores on the SIMS and “perceived health” of the individual over himself. The results significantly discriminated individuals, obtaining higher scores in SIMS those who did not consider themselves “healthy". No significant impacts were identified sociodemographic variables (age, gender and education level) on the overall outcome of the SIMS. All in all, our findings provide a basis for the usefulness of the SIMS as a screening tool for malingering. Implications for forensic practice are discussed and recommendations for future research are offered. |
