Title: Falsos positivos no teste de rastreio para o VIH

Abstract: O número de indivíduos infetados pelo VIH têm vindo a aumentar ao longo dos anos. Perante a gravidade e o estigma criado em torno desta doença, há um crescente interesse na compreensão da ocorrência de falsos positivos nos testes de rastreio para o VIH, dada a implicação deste resultado na vida de um indivíduo. Assim, este trabalho foca-se na análise dos resultados falsos positivos obtidos no teste de rastreio para o VIH no sentido de estudar a prevalência, destes resultados na população estudada, relacionar os dados demográficos (género e faixa etária), dados clínicos (diagnóstico, antecedentes pessoais, medicação crónica, possíveis fatores de risco), bem como alguns dos parâmeros laboratoriais. Além disso, pretende analisar mais pormenorizadamente a população de grávidas e os resultados do teste do CItomegalovírus. Os resultados obtidos evidenciaram que os indivíduos que apresentavam um resultado falso positivo eram maioritariamente do sexo feminino e compreendidos entre os 21-30 e 51-60 anos, parecendo ser predominante na idade fértil e no início da menopausa. Os indivíduos do sexo masculino predominam em idades muito jovens e muito avançadas. Em 24% dos indivíduos, a condição mais associada aos falsos positivos, foi a gravidez. Os problemas cardiovasculares e respiratórios foram os mais frequentes na população de falsos positivos, assim como as alterações laboratoriais observadas eram indicativas de doença hepática. Na avaliação da performance dos métodos usados observou-se um maior número de falsos positivos nos resultados por quimioluminescência do que por ELFA. Relativamente à prevalência dos resultados do CMV verificou-se que alguns indivíduos apresentavam imunidade para o CMV e apenas um doente apresentou infeção aguda pelo CMV. Verifica-se que o número de falsos positivos (0,46%) no teste de rastreio pelo VIH analisados neste estudo é bastante significativo e parece estar associado a condições como a gravidez e o CMV. Estudos futuros serão importantes para uma melhor compreensão de um resultado falso positivo num teste de rastreio para o VIH, com vista á otimização destes testes e à diminuição dos efeitos subjacentes a um resultado deste tipo.
The numbers of individuals affected by HIV infection have been increasing over the years. In view of the gravity and the stigma created around this disease, there is a growing interest in understanding the occurrence of false positives in diagnosis tests, given the implication of this result on the life of an individual. This work focuses on the analysis of false positive results obtained in the screening test for HIV in order to study the prevalence of these findings in the study population, relate the demographic data (gender and age), clinical data (diagnosis, personal history, chronic medication, possible risk factors) as well as some of the parameres laboratory. It also aims to investigate further the population of pregnant and the test results of cytomegalovirus. It was found that individuals who had a false positive result were mostly female and included among the 21-30 and 51-60 years seems to be predominant in fertile age and early menopause. The males are more common among very young and very advanced. In 24% of individuals the condition most often associated with false positives was pregnant. The cardiovascular and respiratory problems were most frequent in the population, as well as laboratory abnormalities observed were indicative of liver disease. In evaluating the performance of the methods used noted that the number of false positives by chemiluminescence was much higher than for ELFA. For the prevalence of the results of CMV was found that some individuals had immunity for CMV and only one patient acute CMV infection. It is noted that the number of false positives (0.46%) in the screening test discussed HIV is significant in this study and seems to be associated with conditions such as pregnancy and CMV. Future studies will be important for better understanding of a false positive result on a screening test for HIV, with the aim of these tests will optimize and reduce the effects underlying such a result.
