Title: Epidemiologia de matalo-ß-lactamases em isolados clínicos de Pseudomonas aeruginosa resistentes ao imipenemo

Abstract: Pseudomonas aeruginosa é um patogénico humano oportunista responsável por infecções nosocomiais, particularmente em doentes imunocomprometidos. Os carbapenemos são agentes antimicrobianos frequentemente utilizados no tratamento de infecções provocadas por P. aeruginosa. Contudo, nesta espécie, tem-se verificado um aumento de estirpes resistentes a estes antibióticos. A produção de metalo-β-lactamases constitui um dos mecanismos de resistência emergente aos carbapenemos. As metalo-β-lactamases caracterizam-se pela eficiente capacidade de hidrolisar a maioria dos β-lactâmicos, especialmente os carbapenemos. A observação de isolados produtores das metalo-β-lactamases IMP e VIM tem aumentado por todo o mundo. A metalo-β-lactamase VIM é a mais frequente na Europa mediterrânica. Recentemente em P. aeruginosa foram descritas as metalo-β- lactamases SPM-1 no Brasil e GIM na Alemanha. Neste trabalho foram estudados 136 isolados clínicos resistentes ao imipenemo seleccionados a partir de 773 isolados de P. aeruginosa, colhidos durante dois anos (Abril de 2003 e Abril de 2005) no Centro Hospitalar de Coimbra. As susceptibilidades destes isolados resistentes ao imipenemo a outros antibióticos foram: meropenemo 20,3%, ceftazidima 56,7%, cefepima 49,6%, piperacilina/tazobactam 47,4%, aztreonamo 37,3%, amicacina 73,1% e ciprofloxacina 53,0%. O teste de sinergismo do duplo disco foi usado para triagem dos isolados clínicos resistentes ao imipenemo relativamente à presença de metalo-β-lactamases, observando-se que 40 (29,4%) isolados apresentaram teste fenotípico positivo. O gene blaVIM foi detectado em 26 isolados (19,1%). As metalo-β-lactamases IMP, SPM e GIM não foram detectadas. Por sequenciação verificou-se a presença de VIM-2 nestes 26 isolados. As CMIs dos isolados que apresentaram VIM-2 foram determinadas e os resultados revelaram que a piperacilina foi o antimicrobiano mais eficaz (84,6%) seguido do aztreonamo (76,9%), ceftazidima (53,8%) e meropenemo (20,3%). O gene blaVIM-2 é encontrado com uma certa frequência em cassetes genéticas inseridas em integrões de classe 1. Em três isolados blaVIM-2 positivos foram detectados estes integrões, verificando-se que o gene blaVIM-2 estava inserido nesses integrões de classe 1 conferindo-lhes potencial de disseminação. A tipagem por RAPD dos isolados blaVIM-2 positivos demonstrou 7 tipos de perfis (A a G), com predominância do perfil A que se encontrava disseminado por 5 Unidades Hospitalares, nomeadamente Medicina, Reanimação, Pneumologia, Neurocirurgia e Ortotraumatologia. ABSTRACT: Pseudomonas aeruginosa is an opportunistic human pathogen and a leading cause of nosocomial infections, particularly in immunocompromised patients. Carbapenems are important therapeutic agents for the treatment of infection with multidrug-resistant Gram negative bacteria, such as P. aeruginosa. The production of metallo-β-lactamases is one of the emerging mechanisms of carbapenem resistance in P. aeruginosa. The metallo-β-lactamases confer efficient hydrolysis of almost all of β-lactam, especially carbapenems. Clinical isolates harbouring the metallo-β-lactamases IMP and VIM have been increasingly reported worldwide, and VIM has been reported especially in Mediterranean Europe. Recently, in P. aeruginosa another two metallo-β- lactamases have been described: SPM-1 in Brazil and GIM in Germany. For this work, 773 clinical isolates of P. aeruginosa were collected in Centro Hospitalar of Coimbra, during two years (April 2003 to April 2005). From these, 136 (17.6%) isolates were selected because they were resistant to imipenem. The susceptibilities to others antibiotics of imipenem resistant isolates were: meropenem 20.3%, ceftazidime 56.7%, cefepime 49.6%, piperacillin plus tazobactam 47.4%, aztreonam 37.3%, amikacin 73.1% and ciprofloxacin 53.0%. Clinical isolates of P. aeruginosa imipenem resistant were initially screened for the presence of metallo-β-lactamases by a double-disk synergy test. Forty (29.4%) isolates had positive phenotypic screening tests. Twenty-six isolates (19.1%) demonstrated the presence of blaVIM. The others metallo-β- lactamases tested were negative. Sequencing demonstrated the presence of VIM-2 enzyme in these 26 isolates. Among these strains that harboured VIM-2, MICs were determined and the results revealed that piperacillin inhibited 84.6% followed by aztreonam 76.9%, ceftazidime 53.8% and meropenem 20.3%.The blaVIM-2 gene is usually in a gene cassette integrated in class 1 integrons. In the isolates blaVIM-2 positives the class 1 integrons were detected in three isolates. In these isolates blaVIM-2 gene were inserted in class 1 integrons with the potencial to spread. RAPD typing of the isolates blaVIM-2 positives demonstrated seven distinct amplification patterns (A to G), with predominance of pattern A, that was disseminated in 5 wards, namely Medicine, Reanimation, Pneumonology, Neurosurgery and Orthotraumatology.
