Abstract: | Enquadramento: As doenças cardiovasculares são a principal causa
de mortalidade a nível mundial, sendo a hipertensão arterial, um
importante problema de saúde pública. A intervenção precoce é muito
relevante para a prevenção da doença crónica e das suas
complicações, através da promoção da adesão terapêutica.
Objetivos: Identificar os fatores condicionantes dos comportamentos
de adesão da pessoa com Hipertensão Arterial (HTA) que suportem
estratégias de intervenção do enfermeiro de família.
Métodos: Estudo do tipo exploratório e transversal, de natureza
quantitativa, descritivo-correlacional, desenvolvido numa Unidade de
Saúde Familiar (USF) do Agrupamento de Centros de Saúde (ACeS)
do Baixo Vouga, com uma amostra por conveniência, constituída por
286 utentes. Decorreu no âmbito da consulta de vigilância de HTA,
tendo presentes os princípios éticos no que respeita ao consentimento
informado livre e esclarecido dos participantes e ao anonimato e
confidencialidade da informação. Aplicou-se o formulário de Sousa
(2005), relativo à adesão ao regime terapêutico, entre fevereiro e abril
de 2017. Os dados foram tratados através da versão 24 do programa
Statistical Package for the Social Sciences (SPSS®).
Resultados: A amostra é constituída por 51,7% de participantes do
sexo feminino, a idade média foi de 66,51 ± 11,35 anos. A maioria dos
participantes apresenta excesso de peso ou obesidade grau I, um risco
de complicações muito aumentado e 43,7% dos inquiridos apresenta
valores de PA superiores a 140/90 mmHg. A adesão ao regime
terapêutico traduziu-se por uma pontuação mais elevada no que se
refere à toma da medicação e ao abuso de tabaco e mais baixa, no
que se refere à prática de exercício físico. A adesão global mostrou-se
superior nos idosos, nos não ativos profissionalmente, nos que
necessitavam de mais fármacos, nas mulheres e nos que valorizavam
a intervenção do enfermeiro de família.
Conclusão: A adesão ao regime terapêutico da HTA é um grande
desafio ao alcance dos enfermeiros de família, tornando-se uma
necessidade vital investir na literacia e na capacitação dos cidadãos,
para a adoção de estilos de vida promotores da saúde, visando a
obtenção de ganhos em saúde e, por inerência, uma redução dos
custos dos tratamentos. Background: Cardiovascular diseases are the main cause of mortality worldwide, and hypertension is an important public health problem. Early intervention is very relevant for the prevention of chronic disease and its complications, through the promotion of therapeutic adherence. Objectives: To identify the conditioning factors of the adherence behaviours of the person with arterial hypertension that support intervention strategies by the family nurse. Methods: Exploratory, cross-sectional descriptive and quantitative correlational study, developed in the Family Health Unit of the Agrupamento de Centros de Saúde do Baixo Vouga, with a convenience sample of 286 patients. It took place within the scope of the Hypertension Surveillance Consultation, considering ethical principles, regarding free informed consent of the participants and the anonymity and confidentiality of the information. The form of Sousa (2005), regarding adherence to the therapeutic regimen, was applied between February and April 2017. Data were processed through version 24 of the Statistical Package for the Social Sciences (SPSS®) program. Results: The sample consisted of 51.7% female participants, mean age was 66.51 ± 11.35 years. Most of the participants were overweight or obese, with a very high risk of complications and 43.7% of the respondents had BP values above 140/90 mmHg. Adherence to the therapeutic regimen resulted in a higher score for medication and tobacco abuse and lower for exercise. Overall adherence was higher in the elderly, in those who were not professionally active, in those who needed more drugs, in women and in those who positively valued the intervention of the family nurse. Conclusion: Adherence to the therapeutic regimen of hypertension is a major challenge for family nurses, and it is a vital need to invest in the literacy and empowerment of citizens, making them aware of the need to adopt health-promoting lifestyles, in order to obtain gains in health, and, of course, a reduction in treatment costs |
