Title: Estratégias de coping, qualidade de vida e distress psicológico em doentes com cancro ginecológico e da mama

Abstract: O cancro mais comum na mulher é o cancro da mama e é a segunda causa de morte entre mulheres com cancro. Os cancros dos órgãos reprodutivos são o quarto mais comum nas mulheres onde foram estimadas que cerca de 91.730 mulheres foram diagnosticados com algum tipo de cancro ginecológico nos Estados Unidos em 2013. A revelação de um diagnóstico de cancro e os tratamentos necessários ao seu combate podem provocar distress psicológico nas doentes e quando não se utilizam estratégias de coping adaptativas ao combate desse distress as mulheres com cancro ginecológico e da mama podem apresentar uma baixa qualidade de vida. Este estudo teve como objetivo avaliar a relação entre as estratégias de coping (EC), a qualidade de vida (QdV) e o distress psicológico em doentes com estes tipos de cancros. A amostra foi constituída por 31 mulheres com cancro ginecológico e da mama com idades compreendias entre os 32 e os 82 anos (M = 58,23; DP = 13,28) seguidas no Instituto Português de Oncologia de Coimbra Francisco Gentil, E.P.E. Os instrumentos utilizados foram os seguintes: um Questionário Sociodemográfico e Clínico, a Hospital Anxiety Depression Scale (HADS), o European Organization for Research and Treatment of Cancer Quality of Life Questionnaire Core-30 (EORTC QLQ C-30) e o Cancer Coping Questionnaire (CCQ). Os resultados indicaram que não existem diferenças significativas entre a QdV e as EC. Quanto à QdV e a HADS-A, foi possível verificar que estas apresentaram correlações negativas significativas na QdV global e nos funcionamentos físico, emocional e cognitivo, e correlações positivas significativas na fadiga, dor e dificuldades financeiras. Em relação à QdV e HADS-D, foram encontradas correlações negativas significativas na QdV global e nos funcionamentos físico, de papel, emocional, cognitivo e social, e correlações positivas significativas na fadiga e dor. Também foi verificada uma correlação negativa significativa entre a ansiedade e as EC interpessoais. Confirmou-se a existência de relações entre a QdV e o distress psicológico (ansiedade e depressão), neste sentido os resultados sugerem a importância de formar e sensibilizar os profissionais de saúde, de forma a detetar sinais precoces de distress psicológico e desenvolver programas de intervenção que possibilitem responder de forma mais eficaz às necessidades específicas destas doentes, nomeadamente na sua QdV.
The most common cancer in women is breast cancer and it is the second leading cause of death among women with cancer. Cancers of the reproductive organs are the fourth most common in women, where it has been estimated that about 91,730 women were diagnosed with some type of gynaecological cancer in the United States in 2013. The revelation of a cancer diagnosis and the treatment needed to fight it can cause psychological distress in patients and when adaptive coping strategies are not used to combat this distress, women with gynaecological and breast cancer may present low quality of life (QoL). The aim of the presente study was to analyze the associations between coping strategies, quality of life and psychological distress in patients with these types of cancers. The sample was composed of 31 women with gynaecological and breast cancer aged between 32 and 82 (M = 58.23, SD = 13.28) treated at the Francisco Gentil Portuguese Institute of Oncology of Coimbra. The instruments used were the following: Sociodemographic and Clinical Questionnaire, Hospital Anxiety Depression Scale (HADS), European Organization for Research and Treatment of Cancer Quality of Life Questionnaire Core-30 (EORTC QLQ C-30) and Cancer Coping Questionnaire (CCQ). The results indicated that there are no significant differences between the QoL and the coping strategies. The data showed that QoL and HADS-A has significant negative correlations in global QoL and in physical, emotional and cognitive functions, and significant positive correlations in fatigue, pain and financial difficulties. Regarding the QoL and HADS-D, significant negative correlations were found in global QoL and in physical, role, emotional, cognitive and social functions, and significant positive correlations in fatigue and pain. A significant negative correlation between anxiety and interpersonal EC has also been found. The existence of relations between QoL and psychological distress (anxiety and depression) was confirmed and these results suggest the importance of training health professionals on early detection of psychological distress in order to develop intervention programs to respond more effectively to the specific needs of these patients, particularly in their QoL.
